Treatment and follow up results of patients with monosymptomatic enuresis nocturna

Abstract

Objective: To evaluate the treatment results of patients with monosymptomatic enuresis nocturna. Methods: 97 patients with monosymptomatic enuresis nocturna were included. First, motivation and behavioral treatment have been applied to the patients for 3 months. 60-120 mcg desmopressin has been applied to 79 patients who did not want to continue to the motivation and behavioral treatment or who did not have benefit from motivation and behavioral treatment or whose disease have relapsed. It is accepted as “full response” if the number of wetting decreases 90-100%, “middle response” decreases 50-90%, “failed response” decreases less than 50%. Treatment and relapse rates were evaluated based on the response to therapy at the time of controls. Results: Mean age of the patients was 7.75±1.96 (5-12) years. 20 of 97 (21%) patients who have taken motivation and behavioral treatment have been cured. Two (%10) of these patients\' diseases have relapsed. Desmopressin has been applied to79 patients who did not have benefit from motivation and behavioral treatment or whose diseases have relapsed. Full dryness has been provided in 36 (46%) patients who have taken desmopressin and came to the control visit. “Middle response” has been obtained in 22 (28%) patients. “Failed response” has been obtained in 21 (26%) patients. Relapse has been seen in case of quitting desmopressin in 35 (60%) patients who have benefit from desmopressin. Conclusion: Although, Desmopressin is the most preferred method in monosymptomatic enuresis nocturna, because of easy usage and fast response, relapse ratio is high following discontinuation of the drug.

Abstract

Amaç: Monosemptomatik enürezis nokturna\'lı hastalarda, tedavi sonuçlarını değerlendirmek. Yöntemler: Monosemptomatik enürezis nokturna tanısıyla tedavi uygulanan toplam 97 hasta çalışmaya dahil edildi. Hastalara önce 3 aylık motivasyon ve davranışa dayalı tedavi uygulandı. Davranış ve motivasyon tedavisine devam etmek istemeyen, tedaviden fayda görmeyen veya nüks olan toplam 79 hastaya, desmopressin 60-120 mikrogram uygulandı. Gece ıslatma sayısının %90-100 azalması tam yanıt, %50-90 azalması orta derecede yanıt, % 50\'nin altında azalması yanıtsız olarak kabul edildi. Hastaların kontrollerinde, tedaviye alınan yanıta göre kür ve nüks oranları değerlendirildi. Bulgular: Hastaların ortalama yaşı 7.75 ± 1.96 yıl (5-12) idi. Davranış ve motivasyon tedavisi sonrası kontrole gelen 97 hastanın 20\'sinde (%21) kür sağlandı. Kür sağlanan bu hastaların 2\'sinde (%10) nüks gözlendi. Davranış ve motivasyon tedavisinden yanıt alınamayan veya nüks olan 79 hastaya desmopressin tedavisi uygulandı. Desmopressin tedavisi alan ve kontrole gelen 36 (%46) hastada tam kuruluk sağlanmıştı. Hastaların 22\'sinde (%28) orta derecede yanıt elde edildi. Hastaların 21 (%26)\'inde ise yanıt alınamadı. Yanıt alınan hastalarda desmopressin tedavisi kesilince 35 (%60) hastada nüks gözlendi. Hastalarda tedaviyi bıraktıracak herhangi bir yan etki gözlenmedi. Sonuç: Monosemptomatik enürezis nokturnada desmopressin uygulama kolaylığı ve kısa sürede olumlu yanıt alınması nedeniyle, tercih edilse de, tedavi sonrası nüks oranları çok yüksektir.
