Neuro-Behçet disease: Coexistence of transverse myelitis and aseptic meningitis

Abstract

Behçet\'s disease is a disorder that the etiology and pathogenesis is not well known and this episodic disease is characterized with recurrent oral and genital ulcers, panuveitis. The incidence of Behçet\'s disease increased in Mediterranean region, Middle East region, East Asian region, some part of Japan. The prevalence of this disease was 110/100000 in Turkey. If a neurological symptom is added to Behçet\'s disease it is called Neuro-Behçet syndrome. The most seen neurological complication of Neuro-Behçet syndrome is headache. In Behçet\'s syndrome if the neurological symptoms are not completely occur, to diagnose neuro-Behçet may be very difficult. However, sometimes neurological complications can be progress very fast. Especially the formation of transverse myelitis is very rare neurologic condition for this syndrome. Here we presented a 17-year-old girl with Behçet\'s disease; she admitted for her headache and confusion, in lumbar puncture findings we considered aseptic meningitis. 1 day later she complained paraparesis and hypoesthesia under thoracal 5 level, MRI scanning showed transverse myelitis in almost all thoracic levels. We presented this case because coexistence of transverse myelitis and aseptic meningitis is very rare.

Nöro-Behçet: Transvers miyelit ve aseptik menenjit birlikteliği

Abstract

Behçet hastalığı, etiyolojisi ve patogenezi net olarak bilinmeyen, tekrarlayan oral ve genital ülserler ve panüveit ile karakterize epizodik bir hastalıktır. Behçet hastalığının Akdeniz sahilleri, Ortadoğu, Doğu Asya, Japonya\'nın bir kısmında sıklığı artmakla birlikte ülkemizdeki prevalansı 110/100000 olarak tespit edilmiştir. Behçet hastalığının klasik semptomlarına nörolojik bulguların eşlik etmesi durumunda Nöro-Behçet Sendromundan söz edilir. Nöro-Behçet Sendromunun en sık nörolojik bulgusu baş ağrısıdır. Behçet hastalarında tam bir nörolojik tablo gelişmemişse nörolojik komplikasyonların saptanması oldukça zor ise de bazen nörolojik komplikasyonlar hızlı bir şekilde ilerleyebilir. Bu hastalarda transvers miyelit gelişiminin tek nörolojik bulgu olarak ortaya çıkması ender bir durumdur. Burada 17 yaşında Behçet tanısıyla takip edilen, tarafımıza baş ağrısı, bilinç bulanıklığı yüksek ateş şikayetleriyle başvuran bir hasta sunuldu. Hastaya yapılan lomber ponksiyon neticesinde aseptik menenjit düşünüldü, ancak daha sonraki takiplerinde paraparezisi ve seviye veren his kusuru gelişmesi üzerine transvers miyelit tespit edildi. Ayrıcı tanıya yönelik yapılan tetkikler neticesinde nöro-Behçet\'e bağlı aseptik menenjit ve transvers miyelit tanısı konuldu. Bu durumun çok nadir görülmesi nedeniyle bu vaka sunulmuştur.
