Cranium metastasis of a squamous cell carcinoma with extensive bone destruction

Abstract

The musculoskeletal metastasis of the lung cancers is rare and locates usually at the spine, pelvis and femur. The cranial metastasis of lung cancer is very rare and generally in small size. Large bone destruction is not re­ported yet. Fifty six years old male patient was admitted to our hospital with the complaints of cough and respira­tory difficulty. The needle biopsy of the lung revealed the diagnosis of squamous cell lung carcinoma (Grade 4) and the patient underwent palliative radiotherapy. The patient was admitted to our hospital 6 month later with rapidly growing mass lesion on the forehead. The radiological ex­amination showed an extraaxial mass lesion 10x9x8 cm in size, which compressed on the brain tissue and caused large destruction in the frontal bone. The mass lesion was removed surgically, the brain tissue was decompressed and the bone defect was repaired with synthetic material. Although similar cases were reported previously, a lung carcinoma, which caused bone defect in this size, was not reported. For such metastases, removal of the mass lesion and cranioplasty are suggested.

Yassı hücreli akciğer karsinomu\'nun geniş kemik yıkımı yapan kafatası metastazı

Abstract

Akciğer kanserlerinin kas-iskelet sistemine metastazı na­dir görülür ve sıklıkla omurga, pelvis ve femura yerleşirler. Kranium\'a akciğer kanserinin metastazı çok nadir olmakla birlikte genellikle küçük boyutlarda olur. Elli altı yaşında erkek hasta öksürük ve nefes darlığı şikayeti ile müracaat ettiği hastanemizde yassı epitelyum hücreli akciğer kar­sinomu (Evre 4) tanısı konulmuş ve akciğerlere yönelik palyatif radyoterapi görmüştür. Yaklaşık 6 ay sonra kafa­sının ön tarafında hızla büyüyen bir şişlik nedeniyle klini­ğimize müracaat eden hastanın yapılan tetkiklerinde fron­tal kemiği destrükte eden ve beyin dokusuna bası yapan 10x9x8 cm. boyutlarında ekstraaksiyel kitle tespit edildi. Kitle lezyonu ameliyat ile çıkarıldı, beyin dokusu basıdan kurtarıldı ve kemik defekti sentetik materyalle kapatıldı. Daha önce benzer olgular yayınlanmışsa da bu boyutta kemik defekti yapan akciğer karsinomu metastazı hiç bil­dirilmemiştir. Bu tip metastazlarda kitlenin çıkartılması ve kranioplasti yapılması önerilir.
