Antenatal Hidronefrozlu Bebeklerin Doğum Sonrası İzlemi

Abstract

Amaç: Antenatal hidronefroz (AH), gebelikte en sık saptanan fetal anomalidir. Bu çalışmanın amacı AH tanısı alan bebeklerin doğum sonrası sonuçlarını retrospektif olarak değerlendirmektir.
Yöntemler: Çocuk Nefroloji Polikliniğimizde 2018-2022 yılları arasında en az 12 ay süreyle AH tanısıyla takip edilen 110 bebeğin (83 erkek, 27 kız) kayıtları geriye dönük olarak incelendi.
Bulgular: Hastaların erkek/kız oranı 3,1 idi ve 65 hastada (%59,1) fizyolojik hidronefroz saptanırken, 45 hastada (%40,9) altta yatan bir neden olduğu belirlendi. Patolojik hidronefrozlu 45 hastanın 31'inde (%28,2) obstrüktif üropati, 12'sinde (%10,9) vezikoüreteral reflü saptandı. Obstrüktif üropati tanısı alan olguların 27'sine (%24,6) üreteropelvik bileşke darlığı, ikisine (%1,8) üreterovezikal bileşke darlığı ve ikisine (%1,8) posterior üretral valv tanısı konuldu. İki (%1,8) olguda multikistik displastik böbrek tespit edildi. Fizyolojik hidronefrozlu hastaların renal pelvis ön arka çapları, patolojik hidronefrozlulara saptananlara göre anlamlı düzeyde düşüktü (p<0.001). Patolojik hidronefrozlular daha sık idrar yolu enfeksiyon geçirmişti. Hastaların takibinde, obstrüktif üropatili 12 hastaya ve vezikoüreteral reflülü 5 hastaya cerrahi işlem uygulandı.
Sonuç: Çalışmamız antenatal hidronefrozların büyük çoğunluğunun yaşamın ilk yılında kendiliğinden gerilediğini ortaya koydu. Ancak inatçı veya alevlenen hidronefroz durumlarında ileri tetkik ve/veya cerrahi müdahale gerekir.
