The Effect of Partial Edentulism Patterns on Mandibular Radiomorphometric Indices: A Retrospective Radiological Study

Abstract

Objective: To compare mandibular cortical structure among fully dentate individuals and patients with Kennedy Class I/II partial edentulism using the Mandibular Cortical Index (MCI), Mental Index (MI) and Panoramic Mandibular Index (PMI), and to test the dentate/edentulous side difference in Kennedy Class II. Methods: A total of 167 panoramic radiographs (56 fully dentate, 58 Kennedy Class I, 53 Kennedy Class II) were retrospectively evaluated. MCI, MI and PMI were measured bilaterally. Group comparisons used one-way ANOVA/Kruskal-Wallis and Pearson's chi-square test; the paired t-test/Wilcoxon signed-rank test compared dentate and edentulous sides in Kennedy Class II cases. Results: 51.5% of patients were female; mean age was 56.86±15.04 years. Kennedy I and II groups were significantly older than the fully dentate group (p<0.001). Age correlated positively with MCI (r=0.383, p<0.001). MCI distribution differed significantly among groups (p<0.001); C1 predominated in fully dentate patients (67.9%), while C3 frequency increased in Kennedy I (27.6%) and Kennedy II (11.3%). No significant group differences were found for MI, total distance (TD) or PMI (p=0.120, 0.096, 0.227). Sex significantly affected MI and TD, favouring men (p=0.007, p<0.001); PMI showed no difference. No significant dentate/edentulous side difference was found for MI, TD, PMI or MCI (all p>0.05). Conclusion: Overall dental status is significantly associated with mandibular cortical erosion (MCI). However, the effect of unilateral free-end edentulism could not be demonstrated at the side-specific level in this sample; findings should be interpreted considering the limited subgroup sample size.
