Gastro-peritoneo-cutaneous fistula following splenectomy

Abstract

Splenectomy operation is usually indicated for treatment of hematological disorders or splenic trauma. Splenectomy complications including gastric injury and peritoneal abscess formation were rarely reported. Forty seven years old male patient diagnosed with immune thrombocytopenic purpura and had splenectomy operation. Abdominal pain and cutaneous fistula was developed after the operation. Further investigations revealed gastric fistula opening endoscopically and presence of intraabdominal abscess radiologically. Patient was diagnosed with gastro-peritoneocutenous fistula. This case suggests that gastro-peritoneocutenous fistula may present as a rare complication after splenectomy.

Splenektominin nadir bir komplikasyonu: Gastroperitoneakutanöz fistül

Abstract

Splenektomi hematolojik hastalıklar veya travma sonrası uygulanan bir işlemdir. Splenektomi sonrası gastrik yaralanma veya fistül gelişimi nadiren izlenebilir. 47 yaşında erkek hastaya İmmün Trombositopenik Purpura nedeniyle splenektomi uygulanmıştır. Splenektomi sonrası karın ağrısı ve kutanöz fistül gelişen olgunun endoskopik incelemesi gastrik fistül orifisi varlığını göstermiştir. Olguda intraabdominal abse varlığı da saptanarak gastro-peritoneakutanöz fistül tanısı konmuştur. Bu olgu splenektomi sonrası karın ağrısı ve kutanöz fistül gelişen olgularda splenektomi sonrası komplikasyonların araştırılması gerektiğini göstermiştir.
