Bir Kedide Primer Akciğer Tümörüne Bağlı Nörolojik Paraneoplastik Sendrom Olgusu

Abstract

Paraneoplastik sendromlar (PNS), vücutta tümörden uzakta, neoplazinin dolaylı etkileri sonucu ortaya çıkan bir grup

sendromdur. Organizmanın birçok sistemini etkileyen bu sendromlar; nöromuskuler kavşak, beyin ve periferal sinirlerin etkilenmesiyle

ortaya çıkan nörolojik paraneoplastik sendromları (NPNS) da içerir. NPNS, klinik ve subklinik olarak özellikle yaşlı kedi ve köpeklerde

gizli birçok malignitenin tanısında büyük önem taşır. Bu çalışmada, bir kedide primer akciğer tümörüne bağlı NPNS olgusu ele alındı.

Olgumuzu, İstanbul Üniversitesi Veteriner Fakültesi, Araştırma ve Uygulama Hastanesi Cerrahi Kliniği’ne sol arka bacağında topallık

şikayeti ile getirilen, 13 yaşında, erkek, American Shorthair ırkı bir kedi oluşturdu. Hastanın fiziksel muayenesinde plantigrad basış

ve alt motor nöron fonksiyon bozukluğu ile ilişkili tibial paraliz tespit edildi. Hastanın hematolojik muayenesinde ve tüm vertebralara

yönelik manyetik rezonans görüntüleme (MRG) incelemesinde anormal bir bulguya rastlanmadı. Bununla birlikte, göğüs bölgesinin,

radyolojik olarak değerlendirilmesinin ardından, spiral bilgisayarlı tomografi (BT) incelemesinde, sağ akciğer kaudal lob düzeyinde,

en geniş yerinde 3 x 2.8 cm boyutlarına ulaşan kitle lezyonu izlendi. Hastada primer akciğer tümörüne bağlı paraneoplastik periferal

nöropatinin var olduğu düşünüldü. Tanı konulmasından yedi gün sonra hastada solunum yetmezliğine bağlı olarak ölüm şekillendi.

A Case of Neurologic Paraneoplastic Syndrome in a Cat Due to a Primary Lung Tumor

Abstract

Paraneoplastic syndromes (PNS) are a group of syndromes that occur in the body away from the primary tumor, due

to the indirect effects of neoplasia. These syndromes which affect most of the organ systems include neurological paraneoplastic

syndromes (NPNS) which arise once the neuromuscular junction, brain and peripheral nerve are affected. The importance of these

clinical and subclinical NPNSs is to diagnose lots of subtle malignities, especially in old cats and dogs. In the current case, a NPNS

due to a primary lung tumor is described. A 13 – year old, male, American Shorthair cat with lameness on left hind limb was admitted

to the Surgery Clinic of Research and Practice Hospital at the Faculty of Veterinary Medicine at Istanbul University. Plantigrade

locomotion and tibial paralysis associated with lower motor neuron dysfunction were noted in physical examination. No abnormalities

in the examination of haematological results and magnetic resonance imagining (MRI) findings of all vertebral region. However, the

radiological examination of the thoracic cavity and the spiral computerized tomography (CT) findings confirmed the diagnosis of a

mass lesion in size of 3 x 2.8 cm at right caudal lung lobe. Paraneoplastic peripheral neuropathy due to a primary lung tumor was

suggested in the patient. The cat died due to respiratory failure seven days after the diagnosis.
