KOAH Hastalarında Demodex (Acari: Demodicidae) Enfestasyonu ve İmmunosüpresyon ile Arasındaki İlişki

Abstract

Bu çalışma KOAH hastalarında Demodex enfestasyonunun belirlenmesi ve
immünosüpresyon ile arasındaki ilişkinin ortaya konulması amacıyla yapıldı. Çalışmaya
Erzincan Üniversitesi Mengücek Gazi Eğitim ve Araştırma Hastanesi Göğüs
Hastalıkları Polikliniğine başvuran ve KOAH (Kronik Obstrüktif Akciğer
Hastalığı) tanısı konulan 101 hasta ve herhangi bir kronik veya dermatolojik
hastalığı olmayan 80 kontrol dahil edildi. Erzincan Üniversitesi Klinik
Araştırmalar Etik Kurulu’ndan onay (Karar no:
2016-08/07) alındıktan sonra tüm katılımcılara bilgilendirilmiş
onam formu okunarak imzalatıldı.
Katılımcıların sağ ve
sol yanağından Standart Yüzeyel Deri Biyopsi (SYDB) yöntemi ile örnekler
alınarak ışık mikroskobunda incelendi. Çalışmada hastaların %82,2’sinde Demodex folliculorum (ortalama 21,78/cm2),
%40,6’sında D. brevis (ortalama 4.70/cm2) olmak üzere %87,1’inde Demodex spp. (ortalama 22,74/cm2);
kontrollerin ise %25’inde D. folliculorum
(ortalama 5,05/cm2), %12,5’inde D.
brevis (ortalama 1,5/cm2)
olmak üzere %27,5’inde Demodex spp.
(ortalama 5,27/cm2) saptandı. Ayrıca Demodex yoğunluğunun immünosüpresif tedavi alan KOAH hastalarında almayanlara
göre yaklaşık 3,5 kat daha fazla olduğu ve aradaki farkların istatistiki
bakımdan anlamlı olduğu tespit edildi. Sonuç olarak çalışmamızda KOAH
hastalarının kontrollerden daha fazla Demodex
ile enfeste olduğu ve immünosüpresyonun Demodex
yoğunluğunu önemli derecede arttırdığı belirlendi. Dolayısı ile immünosüpresif
tedavi alan hastalarda demodikozise bağlı cilt lezyonlarının gelişebileceği ve
bunun klinikte göz önünde bulundurulmasının faydalı olacağı düşünüldü.
