HEMODYNAMIC GAIN OR EASE OF APPLICATION IN CARDIAC RESYNCHRONIZATION THERAPY?

Abstract

This letter provides constructive feedback on a recent study comparing the effectiveness of invasive hemodynamic methods and Doppler echocardiography in optimizing AV/VV delay in patients undergoing cardiac resynchronization therapy (CRT). The study demonstrated clinical and hemodynamic improvements with both methods, but reported that the invasive method was superior in some parameters. However, short follow-up, lack of detailed analysis by patient subgroups, and operator dependence of the measurements are among the factors that should be considered when interpreting the study. Furthermore, the practical limitations of invasive methods and their impact on patient comfort should be considered. In this context, the importance of personalized optimization strategies in clinical practice is emphasized, and the need for future, larger sample-based, long-term studies is emphasized.

KARDİYAK RESENKRONİZASYON TERAPİSİNDE HEMODİNAMİK KAZANÇ MI, UYGULAMA KOLAYLIĞI MI?

Abstract

Bu mektup, kardiyak resenkronizasyon tedavisi (KRT) uygulanan hastalarda invazif hemodinamik yöntem ile Doppler ekokardiyografi yönteminin AV/VV gecikme optimizasyonundaki etkinliğini karşılaştıran güncel bir çalışmaya yönelik yapıcı değerlendirmeleri içermektedir. Söz konusu çalışma, her iki yöntemin de klinik ve hemodinamik iyileşmeler sağladığını ortaya koymakla birlikte, invazif yöntemin bazı parametrelerde daha üstün sonuçlar verdiğini bildirmiştir. Ancak, kısa takip süresi, hasta alt gruplarına göre detaylı analiz eksikliği ve ölçümlerin operatör bağımlılığı gibi noktalar, çalışmanın yorumlanmasında dikkat edilmesi gereken unsurlar arasında yer almaktadır. Ayrıca invazif yöntemlerin pratik uygulamadaki sınırlılıkları ve hasta konforu üzerindeki etkileri göz önünde bulundurulmalıdır. Bu bağlamda, klinik uygulamalarda kişiselleştirilmiş optimizasyon stratejilerinin önemi vurgulanmakta ve gelecekte daha geniş örneklemli, uzun dönemli çalışmaların gerekliliği ifade edilmektedir.
