TÜMÖR NEKROSİS FAKTOR-α -308(A/G) GEN POLİMORFİZMİNİN BEHÇET HASTALIĞININ AKTİF VE İNAKTİF FAZLARINDA ARAŞTIRILMASI

Abstract

Behçet hastalığı dünyada ilk kez 1937 yılında bir Türk dermatolog olan Prof. Dr. Hulusi Behçet tarafından tekrarlayan oral aft, genital ülserasyon ve hipopiyonlu iridosiklitten oluşan ayrı bir antite olarak tanımlanmıştır. Bu sendroma 1947 yılında Behçet Sendromu, Morbus Behçet veya Behçet hastalığı(BH) denilmiş ve tıp literatürüne geçmiştir. Ailevi vakalar hastalıkta genetik faktörleri akla getirirken coğrafi dağılım ise çevresel etkenlerin rol oynayabileceğini düşündürmektedir. BH kronik zeminde akut ataklarla seyreden ve ataklar sırasında çok zengin sitokin salınmasının izlendiği bir hastalıktır. Çok sayıda proinflamatuvar sitokin, kemokin ve özellikle T helper 1 tipindeki sitokinlerin hastalık aktivitesi ile alakalı olduğu gösterilmiştir. Bizde bu yoldan hareket ederek Behçet hastalığında TNF-α -308(G/A) promotor polimorfizmini aktif, inaktif ve göz tutulumlu fazlarda araştırılmasını amaçladık. Dermatoloji ve Göz Hastalıkları Anabilim Dallarına başvuran 30 Behçet hastası ve 10 kontrolden 2ml EDTA’lı kan örneği alınarak, DNA izolasyonları yapıldı. DNA örneklerinden PCR-RFLP metoduna dayalı olarak gen polimorfizm çalışması yapıldı. ÇalışılanTNF-α -308(A/G) polimorfizmi ile Behçet hastalığı arasında bir ilişki bulunamamıştır. Özellikle Behçet hastalığının TNF-α geni -308(A/G) promotor polimorfizminin istatistiksel değerlendirilmesinde hasta sayısının artırılarak yeniden çalışmanın yapılmasıyla anlamsız bulunan promotor polimorfizminin anlamlı kılınabileceğini düşünüyoruz

Determination Of The Tumour Necrosis Factor- α -308(A/G) Gene Polymorphism In Behcet Patients In Active And Inactive Phase Of The Behcet’s Disease

Abstract

Behcet disease first described in 1937 by Turkish Dermatologist Prof Dr. Hulusi Behcet is characterized as a distinct entity by recurrent oral aphthous and genital ulceration and iridocyclitis with hypopyon. The disease was named Behcet Syndrome, Morbus Behcet or Behcet Disease and took its place in the literature. Whereas familial cases suggest the genetic basis of the disease, geographical distribution of the cases suggests that environmental factors may also play a role in the disease progression. BD is a chronic disorder characterized by acute attacks and simultaneous secretion of cytokines in abundance. Previous studies show that considerable number of proinflammatory cytokines, chemokins and especially type 1 helper T cell (Th1) cytokines have the effect on disease activity. Therefore, the aim of the study is to investigate promoter polymorphism of TNF-α -308(G/A) in the patients with active, inactive clinic and BD symptoms in the eyes. Genomic DNA isolated from 2ml blood sample taken in EDTA containing tube from control groups and BD patients applying to Dermatology and Ophthalmology Departments. Cytokine polymorphisms analyses were performed from DNA samples using PCR based RFLP method. No significant differences were found between control and patients groups in terms of promoter polymorphism of TNF-α -308(G/A) in the study. We believe that new studies with larger number of patients are needed to get significant results in promoter polymorphism of TNF-α -308(G/A)
