Phenomenological Approach to Experiences of Indnvnduals Preperad for Stem Cell Transplantation

Abstract

This phenomenological study was conducted to determine the experiences of patients who had been waiting for stem cell therapy in the Bone Marrow Transplant and Stem Cell Therapy Center of a medical faculty. The study was conducted with 12 patients who had been waiting for transplant and was being prepared for transplant. Patients with no communication problem, psychiatric disorder and hearing problem were recruited to the study. Data was collected through quantitative methods. Also, focusgroup and in-depth interviews were done. It is aimed to get in-depth, detailed and multidimensional quantitative data regarding the experiences, senses and attitudes of the patients to undergo bone marrow transplant. It was found that patients felt shocked when they learned about their diseases; prior to transplant, they were affected physically and psychologically by the chemotherapy subsequent to transplant. However, when they heard about transplant, they became hopeful and wanted to go back to their families after therapy. According to these results, it is suggested that nurses should determine how the life of the patients is affected before stem cell transplant, and provide the patients with necessary psychosocial support.

KÖK HÜCRE NAKLİNİ BEKLEYEN HASTALARIN YAŞAMIŞ OLDUĞU DENEYİMLERE FENOMENOLOJİK YAKLAŞIM

Abstract

Bu çalışma, Tıp Fakültesi Kemik İliği Nakli ve Kök Hücre Tedavi merkezinde kök hücre nakli bekleyen hastaların yaşamış olduğu deneyimleri belirlemek amacıyla fenomenolojik olarak yürütülmüştür. Çalışma Kök Hücre Tedavi merkezi servislerinde nakil olmayı bekleyen, nakil için hazırlık yapılan 12 hasta ile yapılmıştır. Çalışma kapsamına; iletişim sorunu bulunmayan, herhangi bir psikiyatrik hastalığı olmayan, işitme problemi olmayan hastalar dahil edilmiştir. Çalışmada veriler odak grup görüşmesi ve derinlemesine görüşme yoluyla toplanmıştır. Veriler 7 tema altında yapılandırılmıştır: “Hastalığı öğrenmek”, “Fiziksel semptomlar ve hasta deneyimleri”, “Hastaların anlam dünyası”, “Bir umut: kök hücre nakli”, “Stresle baş etme yöntemleri”, Sosyal destek “Kök hücre nakli sonrası. Görüşmede kemik iliği transplantasyonu yapılacak hastaların deneyimleri, duyguları ve tutumlarına dair derinlemesine, detaylı ve çok boyutlu nitel bilgi edinilmesi amaçlanmıştır. Çalışmamızda hastaların hastalık tanısını aldıklarında şok yaşadıkları, nakil öncesinde uygulanan kemoterapi tedavisi nedeniyle bireylerin fiziksel ve psikolojik olarak etkilendiği, nakil haberini aldıklarında hastalıklarına bir umut ışığı oluştuğu, tedaviden sonra iyileşerek ailelerine dönmek istedikleri belirlenmiştir. Bu sonuçlar doğrultusunda hemşirelere kök hücre nakli öncesinde hastanın yaşamının hangi yönlerden etkilendiğini belirlemeleri ve hastalara psikososyal yönden destek sağlanması önerilebilir.
