Dikkat Eksikliği Ve Hiperaktivite Bozukluğu Tanılı Bir Vakada Ayres Duyu Bütünleme Temelli Ergoterapi Müdahalesinin Etkileri

Abstract

Amaç: Çalışmamızda dikkat eksikliği ve hiperaktivite bozukluğu (DEHB) tanılı 8 yaşındaki bir çocukta Ayres Duyu Bütünleme® (ADB) temelli ergoterapi müdahalesinin etkinliğinin araştırılması amaçlanmıştır.
Materyal-Metot: Çalışma veriye dayalı karar verme sistemine göre desenlenmiştir. Aile ile yapılan kapsamlı görüşme ve gözlem sonrasında birey standart ölçek ve testlerin yapılması için yönlendirilmiştir. Müdahale öncesinde Duyu Profili (DP) ölçeği ve Duyusal İşlemleme Ölçeği-Ev Formu (DİÖ) ile Duyu Bütünleme ve Praksi Testi (DBPT) kullanılarak duyusal işlem ve praksi becerileri değerlendirilmiştir. Kapsamlı değerlendirme sonucunda elde edilen veriler ile oluşturulan hipotez sonrasında, bireyin günlük yaşamındaki beş farklı alana yönelik hedefi içeren Amaca Ulaşım Skalası (AUS) oluşturulmuştur. ADB temelli ergoterapi müdahalesi, haftada iki kere 45 dakikalık bireysel seanslar şeklinde 6 ay boyunca ADB konusunda eğitimli bir ergoterapist tarafından uygulanmıştır. Müdahalenin ADB’ye uygunluğu Ayres Duyu Bütünleme Uygunluk Ölçütü (ADBUÖ) kullanılarak değerlendirilmiştir.
Bulgular: Müdahale ADBUÖ süreç elementlerinde ortalama 85.16 puan alarak ADB kriterlerini karşılamıştır. Altı aylık ADB müdahalesi sonrasında aile ve okuldaki sınıf öğretmeni şikayetlerin büyük ölçüde azaldığını belirtmiştir. Ayrıca AUS ile belirlenen beş hedefin tamamnında beklenilenin çok üstünde bir seviyeye (+2) ulaşılmıştır. DİÖ ve DP aile ölçeklerinde bütün alt test puanları tipik aralığa kaymıştır.
Sonuç: Çalışmamız DEHB tanılı bir bireyde ADB temelli ergoterapi müdahalesinin duyusal işlem ve katılımı etkileyen faktörler üzerine olumlu etkilerini ortaya koymuştur.
