Acute Pancreatitis due to Mushroom Poisoning: A Case Report

Abstract

Different organ dysfunctions may be seen in mushroom poisoning. However, acute pancreatitis is a rare complication of mushroom poisoning. Sixtysix years old women admitted to emergency department 24 h after wild mushroom ingestion with complaints of nausea, vomiting and abdominal pain. The epigastric discomfort was determined in initial examination. It was determined the elevated serum hepatic transaminases, amylase and lipase levels on admission. Computerized tomography of the abdomen showed minimal peripancreatic fluid. The patient was diagnosed as acute pancreatitis and mushroom poisoning and he was observed in emergency intensive care unit. Hepatic transaminases and pancreatic enzymes levels were progressively decreased and the clinical course was good during the observation period. The patient recovered without complication and was discharged from the hospital. Early symptoms of mushroom poisoning and acute pancreatitis are similar. Possibility of acute pancreatitis as well as other organ dysfunctions should be investigated in patients with mushroom poisoning. Early recognition and appropriate therapy for acute pancreatitis and mushroom poisoning may lead to an improved prognosis and complications.

Mantar Zehirlenmesine Bağlı Gelişen Akut Pankreatit: Olgu Sunumu

Abstract

Mantar zehirlenmelerinde farklı organ yetersizlikleri görülebilir. Ancak akut pankreatit mantar zehirlenmesinin nadir bir komplikasyonudur. Altmış altı yaşında kadın hasta yabani mantarı yemesinden 24 saat sonra bulantı, kusma ve karın ağrısı yakınmaları ile acil servisimize başvurdu. Hastanın fizik muayenesinde epigastrik bölgede hassasiyet vardı. Başvuru anında serum hepatik transaminazlar, amilaz ve lipaz düzeylerinde yükseklik saptandı. Çekilen bilgisayarlı batın tomografisinde peripankreatik mayi görüldü. Hasta mantar zehirlenmesine ve akut pankreatit tanılarıyla acil yoğun bakım ünitesine yatırıldı. Takip süresince klinik durumu iyi olan hastanın hepatik transaminazları ve pankreatik enzim düzeyleri progresif olarak azaldı. Komplikasyon gelişmeyen hasta şifa ile taburcu edildi. Mantar zehirlenmesinin ve akut pankreatitin erken semptomları benzerdir. Mantar zehirlenmeli hastalarda diğer organ yetersizliklerinin yanı sıra akut pankreatit olasılığı düşünülmelidir. Akut pankreatit ve mantar zehirlenmesinin erken tanısı ve uygun tedavisi iyi prognozun sağlanması ve komplikasyonların önlenmesine katkı sağlayabilir.
