EVALUATION OF THE POSITION AND MANDIBULAR CANAL CONDITIONS OF THIRD MOLAR TEETH IN PANORAMIC RADIOGRAPHIES

Abstract

OBJECTIVE: In this study; it was aimed to retrospectively evaluate the position, eruption level and impaction status of impacted third molars according to Winter, Archer, Pell
and Gregory classifications of patients who applied to Gazi University Faculty of Dentistry, Department of Oral and Maxillofacial Surgery between 2018-2019.
MATERIALS AND METHOD: 5523 impacted third molars detected in panoramic films taken from 1548 patients with a mean age of 25.78 were evaluated according to demographic characteristics such as age and gender. All impacted third molars were classified according to their positions and the results were compared with the literature.
RESULTS: According to the analysis results; there was a statistically significant relationship between gender and the positions of teeth 18, 38 and 48 according to the
Winter classification, between gender and the levels of teeth 18, 28 and 48 according to the Archer classification, between gender and the relationship of impacted teeth
with the mandibular canal (p=0.000), however there was no statistically significant relationship between gender and M3 distance according to Pell and Gregory classification
(p>0.05).
CONCLUSION: There are some risks in the extraction of impacted third molars. These risks may arise from the impacted state and position of the teeth. Preoperative evaluation is essential in order to identify and minimize possible risks, especially in terms of the relationship of impacted lower third molar roots with the mandibular canal.

PANORAMİK RADYOGRAFİLERDE ÜÇÜNCÜ MOLAR DİŞLERİN GÖMÜKLÜLÜK VE MANDİBULAR KANAL İLE İLİŞKİ DURUMLARININ DEĞERLENDİRİLMESİ

Abstract

AMAÇ: Bu çalışmada; 2018-2019 yılları arasında Gazi Üniversitesi Diş Hekimliği Fakültesi Ağız, Diş ve Çene Cerrahisi Anabilim Dalı’na başvuran hastaların gömülü üçüncü molar dişlerinin; Winter, Archer ile Pell ve Gregory sınıflandırmalarına göre pozisyonlarının, sürme seviyelerinin ve gömüklülük durumlarının retrospektif olarak değerlendirilmesi amaçlanmıştır.
GEREÇ VE YÖNTEM: Yaş ortalaması 25.78 olan 1548 hastadan alınan panoramik filmlerde tespit edilen 5523 gömülü üçüncü molar dişi, yaş ve cinsiyet özelliklerine göre
değerlendirilmiştir. Tüm gömülü üçüncü molar dişlerin pozisyonlarına göre sınıflandırılması yapılmış ve elde edilen sonuçlar literatür ile karşılaştırılmıştır.
BULGULAR: Analiz sonuçları doğrultusunda; cinsiyet ile Winter sınıflamasına göre 18, 38 ve 48 nolu dişlerin pozisyonları arasında, cinsiyet ile Archer sınıflamasına
göre 18, 28 ve 48 nolu dişlerin sürme seviyeleri arasında; cinsiyet ve 38-48 nolu gömülü dişlerin mandibular kanal ile olan ilişkisi arasında istatistiksel olarak anlamlı bir ilişki
olduğu (p=0.000), cinsiyet ile Pell & Gregory sınıflamasına göre 38-48 nolu gömülü dişlerin M3 mesafesi arasında istatistiksel olarak anlamlı bir ilişki olmadığı tespit
edilmiştir (p>0.05).
SONUÇ: Gömülü üçüncü molar dişlerin çekiminde bazı riskler bulunmaktadır. Bu riskler dişlerin gömüklülük durumundan ve pozisyonlarından kaynaklanabilir. Özellikle gömülü alt üçüncü molar diş köklerinin mandibular kanal ile olan ilişkisi açısından olası risklerin tespit edilerek en aza indirilmesi amacıyla preoperatif değerlendirme yapılması elzemdir.
