Abstract

Objective: Carcinoid tumors occur in children under 15 years of age with an incidence of 1 to 1.4 cases per 1,000,000 population. Although carcinoid tumors are seen in the lungs and some parts of the gastrointestinal tract, they mostly occur in the appendix. This stud retrospectively analyzed the epidemiological and clinical characteristics and treatments of carcinoid tumors patients followed up in our clinic.Material and Methods: This study used a retrospective study design to examine the files of patients diagnosed with carcinoid tumors and treated in the Pediatric Oncology Clinic of Selçuk University Faculty of Medicine between 2010 and 2017. Results: 10 carcinoid tumor patients who were diagnosed, treated and followed up were included in the study. Among the patients, seven were female 70% and three were male 30% . The age of the patients ranged from 10 to 17 years median age: 15 years . Tumor locations were the appendix n: 9, 90% and the lung n: 1, 10% . All patients with appendiceal carcinoid tumors complained of abdominal pain at admission and had been operated for acute appendicitis. The tumor diameter ranged from 0.3 to 2.5 cm median 0.7 cm . In five patients, the tumor was limited to the appendix, while in two patients, the tumor extended beyond the appendiceal serosa. In the two patients, the tumor diameter was 1.7 cm and 2.5 cm. The patient with a tumor diameter of 2.5 cm underwent a right hemicolectomy operation. All patients with appendiceal carcinoid tumors are followed up without the disease. The patient with a pulmonary carcinoid tumor presented with the complaint of persistent cough and the bronchoscopy showed a carcinoid tumor in the lower lobe of the left lung. This patient underwent a lobectomy. This patient is still followed up without the disease. None of the patients had symptoms of carcinoid syndrome.Conclusion: It is of vital importance to follow up the pathological examination of patients operated for acute appendicitis because carcinoid tumors, which are rare in childhood, are incidentally diagnosed

Çocukluk Çağı Karsioid Tümörleri: Tek Merkez Deneyimi

Abstract

Amaç: Karsinoid tümörler KT , 15 yaş altı çocuklarda 1-1.4/1.000.000 sıklıkta görülmektedir. Akciğer ve gastrointestinal sistemin diğer bölgelerinde de görülmekle beraber çoğunlukla apendikste görülür. Bu çalışmada kliniğimizde KT tanısı ile izlenen hastaların epidemiyolojik ve klinik özellikleri ile tedavileri retrospektif olarak değerlendirildi. Gereç ve Yöntem: Selçuk Üniversitesi Tıp Fakültesi, Çocuk Onkoloji Kliniği’nde 2010-2017 arasında KT tanısı ile tedavi edilen hastaların dosyaları retrospektif olarak incelendi. Bulgular: Tanı alıp, tedavisi ve izlemi yapılan 10 KT hastası çalışmaya dâhil edildi. Hastaların 7’si kız % 70 ve 3’ü erkekti % 30 . Hastaların yaşları 10-17 yıl arasında değişiyordu ortanca yaş:15 yıl . Tümör yerleşimleri apendiks n: 9, % 90 ve akciğerdi n: 1, % 10 . Apendiks kaynaklı KT hastaların hepsinde başvuru şikâyeti karın ağrısı olup akut apendisit tanısıyla ameliyat edilmişti. Tümör çapları 0,3-2,5cm arasında değişiyordu ortanca 0,7 cm . Beş hastada tümör apendikste sınırlı iken2 hastada tümör apendiks serozasını aşmıştı. Bu hastalarda tümör çapı 1,7 cm ve 2,5 cm idi. Tümör çapı 2,5 cm olan hastaya sağ hemikolektomi yapıldı. Apendiks kaynaklı KT hastaların hepsi hastalıksız olarak izlemleri devam etmektedir. Akciğer kaynaklı KT hastası, geçmeyen öksürük şikâyeti ile başvurmuş, yapılan bronkoskopide sol akciğer alt lobda KT saptanmıştır. Bu hastaya, lobektomi yapıldı. Bu hasta da halen hastalıksız olarak izlemi devam etmektedir. Hastaların hiçbirinde karsinoid sendrom bulguları yoktu.Sonuç: Çocukluk çağında nadir görülen karsinoid tümörlerin tanısı insidantel olarak konduğu için akut apandisit tanısı ile ameliyatedilen hastaların patolojik incelemelerinin takibi önemlidir
