Bruselloz Tanısında Yeni Bir Yöntem: Brucella Coombs Gel Test

Abstract

Amaç: Bruselloz Brucella cinsi bakterilerin etken olduğu, özellikle gelişmekte olan ülkelerde insanlarda ve hayvanlarda yaygın olarak görülen endemik bir zoonozdur. Brusellozun tanısında mikroorganizmanın kültürde üretilmesi altın standart olmasına rağmen izolasyon oranı çok düşüktür. Bu nedenle serolojik yöntemler daha sık kullanılmaktadır. Bu çalışmanın amacı yeni geliştirilen Brucella Coombs Gel testi ile brusellozun serolojik tanısında standart yöntem olan Coombs’lu Standart Tüp Aglütinasyon testini STA karşılaştırmaktır.Gereç ve yöntem: Selçuk Üniversitesi Tıp Fakültesi Hastanesi’nin çeşitli kliniklerinde bruselloz ön tanısı alan 74 hastanın serum örneği çalışmaya dahil edildi. STA Spinreact, İspanya testi 1/40-1/5120 arası dilüsyonlarda çalışıldı. 24 saat inkübasyondan sonra aglütinasyon görülmeyen tüplere Coombs testi uygulandı. Brucella Coombs Gel testi firma önerileri doğrultusunda çalışıldı. Her iki testte de 1/160 ve üstündeki titreler pozitif kabul edildi.Bulgular: Çalışmaya alınan 74 serum örneğinin 56’sında %75.7 Coombs’lu STA testi ile 53’ünde %71.6 Brucella Coombs Gel testi ile pozitiflik saptanmıştır. Coombs’lu STA testi ile karşılaştırıldığında Brucella Coombs Gel testinin duyarlılığı %94.6, özgüllüğü %100, pozitif prediktif değeri %100 ve negatif prediktif değeri %85.7 olarak bulunmuştur. Sonuç: Sonuç olarak bu çalışmada, Brucella Coombs Gel testinin brusellozun tanısında ve takibinde kullanılan serolojik testlerden Coombs testi ile benzer performansa sahip olduğu görülmüştür

Abstract

Objectives: Brucellosis caused by Brucella genus is an endemic zoonosis which is widely seen in humans and animals especially in developing countries. Although the growth of the microorganism in culture is a gold standard in diagnosis of brucellosis, the isolation rate is very low. Thus, the serological methods are commonly used. This study aims to compare the newly developed Brucella Coombs Gel test with standard tube agglutination STA test with Coombs that is the standard method in the serological diagnosis of brucellosis.Material and methods: The serum samples of 74 patients diagnosed with brucellosis in the various clinics of Selcuk University, Faculty of Medicine Hospital were included in the study. STA test Spinreact, Spain was studied between 1/40-1/5120 dilutions. After 24 hours of incubation, Coombs test was applied to the tubes invisible agglutination. Brucella Coombs Gel test was performed in accordance with companies recommendations. >1/160 titers were considered as positive in both tests. Results: 56 75.7% and 53 71.6% of 74 patients included in the study were positive in STA test with Coombs and Brucella Coombs Gel test respectively. When compared to STA test with Coombs, the sensitivity, specificity, positive predictive and negative predictive values of Brucella Coombs Gel test were 94.6%, 100%, 100% and 85.7% respectively.Conclusion: Brucella Coombs Gel test has performance similar to Coombs test is serological test used in the diagnosis and monitorng of brucellosis
