Acil müdahale gerektiren özofagus yabancı cisimleri: Alkalin pil yutulması

Abstract

Amaç: Elektronik cihazların yaygın kullanımı ile birlikte çocuklarda alkalin pil yutulması sıklığı artmaktadır. Bu çalışmada ciddi komplikasyonlara yol açabilen bu durumun literatür eşliğinde tartışılması amaçlanmıştır. Gereç ve yöntem: Kliniğimizde 2007-2013 yılları arasında alkalin pil yutulması nedeni takip ve tedavisi yapılan dokuz %5.73 olgu yaş, cinsiyet, hastaneye başvurma ve müdahale süreleri, semptom, lokalizasyon, disk pilin çapı, uygulanan tedavi yöntemi, morbidite ve hastanede kalış süreleri bakımından retrospektif olarak incelendi. Bulgular: Olguların yedisi %77.7 ’si erkek, ikisi %22.3 ’si kız, yaş ortalaması 2.55±1.33 1-5 yaş olarak saptandı. Olguların hastaneye başvurma süreleri ortalama 2.33±1.5 saat olarak belirlendi. Tüm olgulara genel anestezi altında acil olarak rijit özofagoskopi yapıldı. Hastaneye getirilişleri ile rijit özofagoskopi yapılarak yabancı cismin çıkarılması arasında geçen süre 30.55±9.6 dakika idi. Sekiz olguda %88.8 yabancı cisim çıkarılırken bir %11.2 olguda mideye ilerletildi. Endoskopik muayenede bir olguda mukozal erozyon ve yüzeyel ülserasyonlar tespit edildi. Hiçbir olguda morbidite ve mortalite gelişmedi. Sonuç: Alkalin piller çocukluk çağında görülen özofagus yabancı cisimleri içerisinde en dikkat edilmesi ve acilen çıkarılması gereken aksi halde mortal komplikasyonlara yol açabilen cisimlerdir. Alkalin pil yutulması sonucu gelişebilen tehlikelere karşı ailelerin ve sağlık personelinin bilinçlendirilmesi morbidite ve mortalite oranlarının azalmasını sağlayacaktır

Foreign bodies of the esophagus requiring urgent intervention: Swallowed alkaline battery

Abstract

Objectives: The frequency of swallowing alkaline batteries is increasing in children, together with the extensive use of electronic devi- ces. The aim of the present study was to discuss this serious condition that can result in serious complications and a review of current literature. Matherial and methods: The present study retrospectively reviewed the medical records of nine cases 5.73% who were admitted to the hospital and treated for swallowed alkaline batteries between 2007 and 2013, and the cases were evaluated in terms of age, gender, admission and intervention times, symptoms, localization, diameter of disc batteries, treatment method, morbidity, and length of hospital stay. Results: Of the cases, seven 77.7% were boys and two 22.3% were girls, and mean age was 2.55±1.33 1-5 years years. Mean time to hospital admission was 2.33±1.5 hours. All cases underwent urgent rigid esophagoscopy under anesthesia. Mean time from hospital admission to the removal of the foreign body with rigid esophagoscopy was 30.55±9.6 minutes. The foreign body was removed in eight cases 88.8% and pushed into the stomach in one case 11.2% . Endoscopic examination showed mucosal erosion and superficial ulcerations in one patient. Morbidity or mortality did not occur in any of the patients. Conclusion: Swallowed alkaline batteries encountered in the childhood are the most significant objects among the foreign bodies of the esophagus that need to be removed urgently due to fatal complications. Raising awareness of the families and health care professionals about the hazards of swallowing alkaline batteries would reduce morbidity and mortality rates
