Kolorektal kansere sekonder gelişen kolokolik invajinasyon

Abstract

Altmış beş yaşında yetişkin bir hastada kolorektal kansere bağlı invajinasyon gelişimi oldukça nadirdir. Bu yazının amacı, invajinas- yonun yetişkinilerdeki insidansını bildirmek, değişken bulguları ve malign hastalıklarla yüksek birlikteliği hakkında bilgi vermek ve tedavi yöntemleri hakkında önerilerde bulunmaktır. Hasta; kilo kaybı, bulantı, kusma, hafif sol taraflı karın ağrısı ve rektal kanama yakınmaları ile acil serviste görüldü. Karın muayenesinde; minimal distansiyon, sol alt kadranda hassasiyet ve kitle saptandı. Digital rektal muayenede rektum boş olup hematokezya mevcuttu. Ayakta direkt batın grafisinde hava-sıvı seviyesi mevcuttu. Batın tomogra- fisinde karın sol tarafta kolokolik invajinasyon saptandı. Hastaya anterior rezeksiyon işlemi uygulandı. Patolojik tanı kötü differansiye stage 2 adenokarsinom olarak raporlandı. Bu vaka karın ağrısı ve rektal kanama şikâyeti olan yetişkin hastalarda ayırıcı tanıda kolo- rektal kansere bağlı invajinasyonun olabileceğini göstermektedir

Colocolic invagination formed secondarily to colorectal cancer

Abstract

The case of a 65-year-old adult patient with intussusception formation related to colorectal cancer is very rare. The aim of this article is to report; the incidence of intussusception in adults, to offer information on variant symptoms and high co-existence with malign diseases, and to suggest treatment methods. The patient presented to the emergency department with complaints of weight loss, nausea, vomiting, mild abdominal pain on the left and rectal bleeding. The abdominal examination of the patient revealed minimal distension, sensitivity in the lower left quadrant and a mass. Digital rectal examination showed that the rectum was empty and there was hematochezia. The patient’s standing direct abdominal graphy revealed air-fluid level. Abdominal tomography showed colocolic intussusception on the left side of the abdomen. Anterior resection was performed on the patient. The pathological diagnosis was reported to be poorly differentiated Stage 2 adenocarcinoma. This case demonstrates that there might be intussusception related to colorectal cancer in the differential diagnosis of patients presenting with complaints of abdominal pain and rectal bleeding
