Semptomatik Rotator Kaf Tendinopatili Bireylerde Dinamik Bantın Akromiohumeral Mesafe, Omuz Propriosepsiyonu ve Eklem Hareket Açıklığı Üzerindeki Akut Etkisinin Araştırılması

Abstract

Giriş: Dinamik bant 2009 yılında kas-iskelet sistemi fizyoterapisti Ryan Kendrick tarafından geliştirilen, dört yönde ve %200'den fazla esneme özeliği olan elastik terapötik bir banttır. Tedavide nörofizyolojik etkisinin yanı sıra güçlü biomekanik etkisi bulunmaktadır. Amaç: Bu araştırmanın amacı, semptomatik rotator kaf tendinopatili (RKT) bireylerde semptomatik omuza uygulanan dinamik bantın akromiohumeral mesafe, omuz eklem hareket açıklığı ve propriosepsiyon üzerindeki akut etkisini araştırmaktı. Yöntem: Çalışmaya RKT’li, yaşları 34-65 arasında değişen ve yaş ortalamaları 54.35±8.04 olan 17 birey dahil edildi. Akromiohumeral mesafe (AHM) Ultrason cihazı ile omuz nötral ve 60° abduksiyon pozisyonunda değerlendirildi. Omuz eklemi propriosepsiyon duyusu dijital inklinometre ile aktif repozisyon testi kullanılarak 30° ve 60° omuz fleksiyon açılarında değerlendirildi. Omuz ağrısız ve tam eklem hareket açıklığı (fleksiyon, abduksiyon, internal ve external rotasyon) standart gonyometre ile değerlendirildi. Bütün değerlendirmeler bantlama öncesi ve sonrası (30 dakika) olmak üzere 2 kez tekrar edildi. Karşılaştırmalarda normal dağılım gösteren veriler için eşleştirilmiş t testi, normal dağılım göstermeyen veriler için Wilcoxon testi kullanıldı. Bulgular: Bantlama öncesi ve sonrası AHM nötral (0°) ve 60º abduksiyon ölçümlerinde istatistiksel olarak anlamlı fark vardı (p<0,001) ve bantlama sonrası AHM değeri daha yüksekti. Benzer şekilde olguların bantlama öncesi ve sonrası propriosepsiyon ölçümleri istatistiksel olarak anlamlıydı (p<0,001). Ağrısız ve tam omuz eklem hareket açıklığı ölçümlerinde ise istatistiksel olarak anlamlı fark yoktu. Sonuç: Bu çalışmada elde edilen verilere göre dinamik bantın semptomatik RKT olan bireylerde akut dönemde AHM’yi arttırmak ve propriosepsiyonu iyileştirmek için etkili bir yöntem olduğu görülmüştür. Omuz ağrısız ve tam eklem hareket açıklığı üzerinde ise değişiklik yaratmadığı tespit edilmiştir

INVESTIGATION OF THE ACUTE EFFECTS OF DYNAMIC TAPE ON ACROMIOHUMERAL DISTANCE, SHOULDER PROPRIOCEPTION AND RANGE OF MOTION IN INDIVIDUALS WITH SYMPTOMATIC ROTATOR CUFF TENDINOPATHY

Abstract

Backgraund: Dynamic tape is an elastic therapeutic tape developed in 2009 by Ryan Kendrick, a musculoskeletal system physiotherapist, with four directions and more than 200% stretch. In addition to its neurophysiological effect, it has a strong biomechanical effect in the treatment. Aim: The aim of this study was to investigate the acute effect of dynamic tape applied to the symptomatic shoulder on acromiohumeral distance, shoulder range of motion, and proprioception in individuals with symptomatic rotator cuff tendinopathy (RCT). Method: Seventeen individuals with RCT, aged between 34-65 and a mean age of 54.35±8.04, were included in the study. Acromiohumeral distance (AHD) was evaluated with the ultrasound device when the shoulder was in neutral and 60° abduction position. The sense of proprioception of the shoulder joint was evaluated at 30° and 60° shoulder flexion angles using the active reposition test with a digital inclinometer. The painless and full range of motion of the shoulder (flexion, abduction, internal and external rotation) was evaluated with a standard goniometer. All evaluations were repeated twice, before and after taping (30 minutes). Paired sample t-test was used for normally distributed data in comparisons, and Wilcoxon test was used for non-normally distributed data. Results: There was a statistically significant difference in AHD neutral (0°) and 60º abduction measurements before and after taping (p<0.001), and the AHD value was higher after taping. Similarly, proprioception measurements of the cases before and after taping were statistically significant (p<0.001). There was no statistically significant difference in painless and full shoulder range of motion measurements. Conclusion: According to the data obtained in this study, it was seen that dynamic tape is an effective method to increase AHD and improve proprioception in the acute period in individuals with symptomatic RCT.
