Abstract

Lomber ponksiyon (LP), tanı ve tedavi amacıyla L3-4 veya L4-5 vertebral aralıktan subaraknoid boşluğa girilerek yapılan bir işlemdir. LP'nin sık karşılaşılan komplikasyonları; baş ağrısı, sırt ağrısı, enfeksiyon, bacaklarda güçsüzlük, subdural hematom, kanama, ponksiyon yerinden BOS sızıntısı, sinir hasarı ve beyin herniasyonudur. Post dural ponksiyon baş ağrısı (PDPB), LP sonrası sık rastlanan komplikasyonlardandır. Görülme sıklığı %10-80 arasında değişmekte ve genellikle girişim sonrası yedi gün içinde ortaya çıkmaktadır. PDPB’de hastaların yarısında LP’den en az bir hafta sonraya kadar günlük yaşam aktivitesinde, iş üretkenliğinde bozulma ve yaşam kalitesinde azalma olmakta, öğrenme yeteneklerinde ve aile içi ilişkilerinde de bozulmalar meydana gelebilmektedir. PDPB’de durumun şiddetine göre tedavi uygulanmaktadır. PDPB’nin geleneksel tedavisinde, yatak istirahati, sıvı tedavisi, analjezik uygulaması, kafein ve epidural kan yaması (EKY) bulunmaktadır. LP, işleminde hemşirelerin işlem öncesinde, işlem sırasında ve sonrasında çeşitli görevleri bulunmaktadır. Bu çalışmanın amacı PDPB’da hemşirelik yaklaşımlarını değerlendirmektir. Bu kapsamda hemşireler, LP işleminden önce hastalara eğitim vermeli, işlem sonrası hastanın karşılaşabileceği komplikasyonları ve bunlarla nasıl mücadele edilebileceği hakkında hastaları bilgilendirmelidir. İşlem sonrasında hastanın dört saat düz bir şekilde yatmasını sağlamalı ve vital bulguları değerlendirilmelidir. Girişim bölgesinde ağrı, şişlik, kızarıklık, kanama, akıntı varlığı değerlendirilmeli ve hasta LP komplikasyonları açısından gözlemlenmelidir.

Nursing Approaches in Post Dural Puncture Headache

Abstract

Lumbar puncture (LP) is a procedure performed by entering the subarachnoid space through the L3-4 or L4-5 vertebral space for diagnostic and therapeutic purposes. Common complications of LP; headache, back pain, infection, weakness in the legs, subdural hematoma, bleeding, CSF leakage from the puncture site, nerve damage, and brain herniation. Post dural puncture headache (PDPB) is one of the common complications after LP. Its incidence varies between 10-80% and usually occurs within seven days after the intervention. In PDPB, half of the patients experience deterioration in daily living activities, work productivity and quality of life until at least one week after LP, and deterioration in learning abilities and family relationships may occur. In PDPB, treatment is applied according to the severity of the condition. The traditional treatment of PDPB includes bed rest, fluid therapy, analgesic administration, caffeine, and epidural blood patch (EBP). In the LP procedure, nurses have various duties before, during and after the procedure. The aim of this study is to evaluate nursing approaches in PDPB. In this context, nurses should educate patients before the LP procedure and inform patients about the complications that the patient may encounter after the procedure and how to combat them. After the procedure, the patient should lie flat for four hours and vital signs should be evaluated. The presence of pain, swelling, redness, bleeding, discharge in the intervention area should be evaluated and the patient should be observed for LP complications.
