YOĞUN BAKIM MALİYETLERİ VE YOĞUN BAKIM MALİYET ÇALIŞMALARININ İNCELENMESİ

Abstract

Bu çalışmada, ulusal ve uluslararası literatürde nitel ve nicel araştırma türünde yapılan yoğun bakım maliyet çalışmalarının mevcut durumunun analizinin ortaya konulması amaçlanmıştır. Araştırma nitel araştırma yöntemi kapsamında gerçekleştirilmiştir. Araştırmada verilerin toplanmasında doküman incelemesi kullanılmıştır. Literatürde yoğun bakım maliyet çalışmaları kapsamında yapılan 240 çalışma araştırma kapsamına alınmıştır. Verilerin analizinde içerik analizi yöntemi kullanılmıştır. Araştırma konusu hakkında genel bir bilgiye sahip ve nitel araştırma yöntemleri konusunda uzman beş kişinin görüşlerine başvurularak araştırmanın inanırlığı arttırılmıştır. Araştırma sonuçlarına göre nicel araştırma yöntemi kapsamında yapılan çalışmaların çoğunluğu oluşturduğu; enfeksiyon, ilaç, sağlık personeli ve kronik hastalık araştırma konularında daha fazla araştırmanın yapıldığı görülmüştür. Veri analiz yöntemi olarak ekonomik analiz ve maliyet etkililik analizi daha çok tercih edilmiştir. Yatılan gün sayısının uzaması ve ilaçların yoğun bakım maliyetini arttırdığı; teletıp ve parenteral beslenmenin ise yoğun bakım maliyetini azalttığı görülmüştür. Ayrıca geri ödeme kurumu tarafından yapılan ödemelerin ve hemşire takip programının yoğun bakım maliyetini hem arttırdığı hem de azalttığı görülmüştür. Araştırma sonuçlarına göre yoğun bakım maliyetlerindeki değişmenin hangi yoğun bakım ünitesinin neden olduğunu belirlemeye yönelik nicel çalışmaların yapılması önerilmektedir.
