Otolog Kök Hücre Nakli Olan Hastanın Dorothy Johnson’ın Davranışsal Sistem Modeline Göre Değerlendirilmesi: Olgu Sunumu

Abstract

Multipl miyelom, plazma hücrelerinin kontrolsüz çoğalmasıyla karakterize, kemik iliğini tutan ve monoklonal immünglobulin üretimiyle seyreden malign bir hematolojik neoplazidir. Multipl miyelom hastalarında kemoterapi sonrası otolog hematopoetik kök hücre nakli standart tedaviyi oluşturmaktadır. Otolog kök hücre nakli, hastanın kendi kök hücrelerinin toplanarak yüksek doz kemoterapi sonrası reinfüzyonuyla hematolojik rekonstitüsyonun sağlandığı bir tedavi yöntemidir. Hemşirelik bakımı sağlamada teori/model kullanımı oldukça önemlidir. Hemşirelik kuramlarının uygulamada kullanımı; nedeni bulma, eleştirel düşünme, karar verme aşamalarında hemşirenin bakış açısını geliştirmektedir. Literatürde yer alan modellerden biri de Johnson'ın “Davranışsal Sistem Modeli”dir. Bu çalışmada, otolog kök hücre nakli olmuş Multipl miyelom tanılı hastanın hemşirelik bakımı Johnson’ın Davranışssal Sistem Modeli kullanılarak sunulmuştur. Olgumuzda öncelikle ayrıntılı öykü alınıp fizik muayene yapılarak modeldeki alt sistemlerin dengesini bozan durumlar belirlenmiştir. Daha sonra dengeyi sağlamak için davranış değişikliği sağlamak üzere uygulanabilecek girişimler planlanmıştır. Kronik bir hastalık olan kanser ve kanser tedavisinde önemli bir yöntem olan otolog kök hücre naklinde davranış değişikliği geliştirmek oldukça önemlidir. Bu anlamda hematolojik kanser hastalarının yönetiminde Johnson’ın Davranış Sistemi Modeli'nin kullanılması ve farklı kronik hastalıklar üzerinde de çalışmaların yapılması önerilmektedir.

Evaluation of a Patient with Autologous Stem Cell Transplantation According to Dorothy Johnson's Behavioral System Model: Case Report

Abstract

Multiple myeloma is a malignant hematologic neoplasm characterized by uncontrolled proliferation of plasma cells that infiltrate the bone marrow and produce monoclonal immunoglobulins. In Multiple myeloma patients, autologous hematopoietic stem cell transplantation following chemotherapy constitutes the standard treatment. Autologous stem cell transplantation is a therapeutic procedure in which the patient's own stem cells are collected and reinfused after high-dose chemotherapy to achieve hematopoietic reconstitution. The use of theory/model is very important in providing nursing care. The use of nursing theories in practice improves the nurse's perspective in the stages of finding the cause, critical thinking and decision making. One of the models in the literature is Johnson's "Behavioral System Model". In this study, the nursing care of a patient diagnosed with Multiple myeloma who received autologous stem cell transplantation was presented using Johnson's Behavioral System Model. In our case, firstly, a detailed history was taken and physical examination was performed and the conditions that disrupted the balance of the subsystems in the model were determined. Then, interventions that could be applied to provide behavioral change to ensure balance were planned. It is very important to develop behavioral change in cancer which is a chronic disease and hematopoietic stem cell transplantation which is an important method in cancer treatment. In this sense, it is recommended to use Johnson's Behavioral System Model in the management of hematologic cancer patients and to conduct studies on different chronic diseases.
