Hall’ın Öz-Bakım-Tedavi Kuramı Bakış Açısıyla Overyan Hiperstimülasyon Sendromu (OHSS)

Abstract

GİRİŞ: Bu çalışmanın amacı overyan hiperstimülasyon sendromu tanısı alan bir olguyu Hall’un öz-bakım-tedavi kuramı bakış açısıyla değerlendirmektir. Hall’un hemşirelik kuramı; bakım (care-hastanın bedeni), öz-çekirdek-merkez (core-bireyin kendisi) ve tedavi(cure-bedeni etkileyen hastalık) olmak üzere üç bileşenden oluşmaktadır. BULGULAR: 35 yaşında, dört yıldır infertil, daha önce infertiliteye yönelik tedavi almamış olan AS,eşiyle birlikte tüp bebek kliniğine başvurmuştur. Yapılan tanısal işlemler sonucunda bilateral polikistik over görünümü ve fallop tüpünün kapalı olduğu belirlenmiştir. Çifte in-vitro fertilizasyon ve embriyo transferi yapılmasına karar verilmiş, HCG uygulamasından 36 saat sonra oosit aspirasyonu gerçekleştirilmiştir. AS oosit aspirasyonundan beş gün sonra ani başlayan göğüs ağrısı, nefes darlığı, ayaklarda ödem, karında ağrı ve şişkinlik şikâyetleriyle kliniğe başvurmuş, tetkikler sonucunda OHSS tanısı konulmuştur. Olguda, kuramın öz-çekirdek-merkez bileşeni olarak AS’ye; kendisine hangi işlemlerin yapılacağı, işlemlerin olası yan etkileri açıklanmıştır. Kuramın bakım bileşeninde vital bulguların takibi, günlük kilo takibi, karın çevresi ölçümü ile hastanın göğüs filmi, EKG ve laboratuvar bulgularının takibi yapılmıştır. Kuramın tedavi bileşeninde ise mobilizasyonuna yardım edilmiştir. SONUÇ: Hall’un öz-bakım-tedavi kuramına temellenen hemşirelik girişimleri ile hasta merkezli bakım gerçekleştirilmiş veklinik semptomların yönetimi gerçekleştirilmiştir. BACKGROUND:The purpose of the study is to evaluate the Overian Hyperstimulation Syndrome from the perspective of Hall's self-care and treatment theory. Hall's nursing theory composes of three parts;care (patient’s body),core (patient himself/herself),cure(sickness that affects a body). RESULTS:A.S.,who had been infertility story for four years,was 35 years old,had never taken treatment before,had applie dto in vitro fertilization clinic due to pregnancy desire.It had been identified that bilateral polycystic over view and closed right fallopian tube.It had been decided to in vitro fertilization and doing embryo transfer,36 hours after GnRHagonist, oocyte aspiration had been done.Five days later from oocyte aspiration,she had consulted the clinic because of chest pain that are sudden onset,shortness of breath,leg odema,stomachache and swelling.A.S. was diagnosed OHSS.Oncase, as doing nursing attempt that are point of view of theory's core component,A.S. had been explained which operation would be done to herself,possible side effects of operations.At the care component of the theory,following up vital sign,daily weight loss,ECG and laboratory findings had been done.At the cure component of the theory,was helped to mobilization. CONCLUSSION:Nursing interventions based on the self-care-treatment theory of Hall and patient-centered care were performed and management of clinical symptoms were performed.
