Hematopoietik Kök Hücre Nakli Hastalarında Etkileşimsel Eğitim Yaklaşımının Anksiyete Düzeyine Etkisi

Abstract

GİRİŞ: Hematopoetik kök hücre nakli nedeniyle zorlu tedavi sürecinde çoğu hastada anksiyete ve duygu durum bozuklukları ortaya çıkabilmektedir. Hematopoietik kök hücre nakli hemşiresi, hastaları yakından takip etmeli ve tedaviye bağlı gelişebilecek ciddi komplikasyonları ve anksiyeteyi önlemede etkili yaklaşımlar konusunda hasta ve yakınlarını bilgilendirmelidir. Amacımız etkileşimli eğitimin hastaların anksiyete düzeyini azaltmada etkisini anlayıp değerlendirebilmektir.
YÖNTEM: Çalışma Ocak 2016 – Ağustos 2016 tarihleri arasında, bir üniversite hastanesinin erişkin kemik iliği nakli ünitesinde gerçekleştirildi. Çalışmaya kök hücre nakli planlanan 50 hasta alındı. 25 deney ve 25 kontrol olarak iki gruba ayrıldı. Deney grubuna araştırmacılar tarafından nakil süreci ve nakil sürecinde kullanılan malzemeler konusunda etkileşimli eğitim verildi, kontrol grubuna ise kliniğin rutin bilgilendirme yaklaşımı uygulandı.
BULGULAR: Hastaların nakil için yattığı gün %56’sında majör anksiyete vardı, nakil öncesi -1. gün hastaların %58’inde majör anksiyetenin devam ettiği ve nakil sonrası +1. günde sadece %6’sında olduğu saptandı. Nakil sürecinde anksiyete düzeyi her iki grupta benzer seyir gösterdi ve nakil işlemi bitene kadar düşmedi.
SONUÇ: Bilgilendirmelere rağmen hastaların anksiyete düzeyinin her iki grupta da nakil işlemi bitene kadar düşmediği görülmektedir. Bu nedenle hastaya nakil süreciyle ilgili bilgilendirmelerin yapılmasının yanı sıra hastaya yattığı günden itibaren hemşire tarafından hasta bakımı planlanırken hastanın bireyselleştirilmesine özen gösterilmesi yarar sağlayabilir.

Effect of Interactive Educational Approach to Level of Anxiety in Bone Marrow Transplantation

Abstract

Introduction: Anxiety can occur during bone marrow transplantation. The hematopoietic stem cell transplant nurse should inform the patients and their relatives about the possible complications. This study was planned to assess the effectiveness of the interactive educational approach in management of bone marrow transplantation related anxiety.
Method: Fifty patients who underwent stem cell transplantation were enrolled in the study between January 2016 and August 2016. Interactive training was given to the experimental group. Routine information approach of the clinic was provided to the control group.
Findings: Major anxiety was in 56% of patients on the day of hospitalization. Major anxiety continued 1 day prior to transplantation in 58% of the patients. Anxiety level followed a similar course in both groups during the transplantation and it did not resolve until end of the transplantation process.
Conclusions: Anxiety level of the patients did not decrease until the end of transplantation in both groups. For this reason, as well as informing the patient about the transplantation process from the day of hospitalisation, care needs to be taken to repeat the information and pay attention to patients individually in order to be ensure about their understanding of the transplant process.
