Hospital Cost Structures: Methodological Issues and Functional Forms for Estimation

Abstract

Hospital are complex multiproduct organisations. From a metholodological perspective, the multiproduct nature of hospitals constitutes problems for the hospital cost analysis. In the analysis of a hospital cost structure, therefore, appropriate techniques should be used. The development of the neoclassical joint cost function and the complementary development of the flexible functional forms, which are based on the neoclassical theory of firms, have facilitated the analysis of hospital costs. The use of flexible forms allows measuring the scale and scope economies. The neoclassical cost function assumes cost minimisation. But the cost-minimising behaviour is questionable for hospitals. Besides, the more recent studies seem to agree that hospitals generally do not operate in their long-run equilibrium and therefore it is more appropriate a short-run cost function.

HASTANE MALİYET YAPILARI: YÖNTEMSEL KONULAR VE TAHMİNE YÖNELİK FONKSİYONEL FORMLAR

Abstract

Hastaneler çok ürünlü organizasyonlardır. Metodolojik yönden bakıldığında hastane maliyet analizlerinde, hastanelerin çok ürünlü niteliği sorunlar yaratmaktadır. Bu nedenle hastanelerin maliyet yapıları ile ilgili analizler yapılırken uygun tekniklerin kullanılması gerekmektedir. Neoklasik bileşik maliyet fonksiyonu ve neoklasik firma tweorisine dayanan esnek fonksiyonel formların geliştirilmesi ile birlikte hastane maliyet analizleri kolaylaşmıştır. Esnek fonksiyonel formların kullanılması ölçek maliyet minimizasyonu varsayımına dayanmaktadır. Ancak hastaneler için maliyet minimizasyonu davranışı tartışmalıdır. Bunun yanında son dönemde yapılan bir çok çalışma hastanelerin uzun dönemde dengede olmadığını varsaydığından kısa dönem hastane maliyet fonksiyonu tahmini yapmak daha uygun görülmektedir.
