Abstract

Cost-Effectiveness Analysis of Contraceptives in Turkey The aim of this study is to evaluated the cost-effectiveness of the contraceptives which provided to people free of charge by Ministry of Health or price of which can be paid by social insurance. The study made from a payers perspective, the data and charges of the year 2008 has been taken into consideration. The study made for 5 years time horizon. The methods related costs have been obtained from the 03.05.2010 dated and 56011 number inscription of Ankara Local Health Department and the data of Health Practice Declaration (SUT) 2007. The data of health segments on which the maintenance are given have been provided 14.01.2011 dated and 49093 numbered inscription of social security association and TNSA 2008 (Population and Health Research of Turkey). The prices except the first years have been taken on discount with ratio of 12% concordance with the revaluation ratio, which was published in 20.11.2008, dated and 27060 numbered official journal. As a result of the base case, analysis IUD (intra uterine device) has been found to be the most costeffective method. The subsequent most cost-effective methods are vasectomy and tubal ligation. IUD has dominated the other methods, which were taken in to consideration. However, all of the methods are cost-effective compared to no method. One-way sensitivity analyses were performed for the time horizon and discount rate. Sensitivity analysis for 7%, 10% and 15% discount rates has been made and the analysis found not to be sensitive to the discount rate. When the period, which the research encloses, changed as 10 years, gradation of costeffectiveness has not been change; only the extent between the cost-effectiveness ratios has been changed

TÜRKİYE’DE AİLE PLANLAMASI YÖNTEMLERİNİN MALİYET-ETKİLİLİK ANALİZİ

Abstract

Amaç: Bu çalısmanın amacı; Türkiye’de Saglık Bakanlıgı tarafından kisilere her hangi bir ücret talep edilmeden sunulan veya bedeli kisilerin baglı oldukları sosyal güvenlik kurumu tarafından ödenen aile planlaması yöntemlerinin (kontraseptifler) maliyet-etkililigini degerlendirmektir. Çalısma geri ödeme kurumu bakıs açısı ile 2008 yılı verileri ve fiyatları esas alınarak yapılmıstır. Arastırmada 5 yıllık zaman dilimi degerlendirilmistir. Etkililik verileri literatürden saglanmıstır. Yöntemlere iliskin maliyetler Ankara l Saglık Müdürlügü’nün 03.05.2010 tarih ve 56011 sayılı yazısı ve Saglık Uygulama Tebligi (SUT) 2007 verilerinden saglanmıstır. Hizmetlerin verildigi saglık basamagına iliskin veriler ise Sosyal Güvenlik Kurumu’nun 14.01.2011 tarih ve 49093 sayılı yazısı ve Türkiye Nüfus ve Saglık Arastırmaları (TNSA) 2008’den yararlanarak bulunmustur. Yöntemlere iliskin maliyet ve etkililik verilerinin modellenmesinde karar agacı yöntemi kullanılmıstır. lk yıl dısında olusacak maliyetler 21.11.2008 tarih ve 27060 sayılı resmi gazetede yayınlanan yeniden degerleme oranına uygun olarak %12 oranı ile iskonto edilmistir. Temel analiz sonucunda en maliyet-etkili yöntemin rahim içi araç (RA) oldugu bulunmustur. RA’dan sonraki maliyet-etkili yöntemler vazektomi ve tüpligasyondur. Degerlendirmeye alınan diger yöntemler ise RA tarafından baskılanmıstır. Ancak yöntem kullanmama ile kıyaslandıgında tüm yöntemlerin maliyet-etkili oldugu bulunmustur. Çalısmanın kapsadıgı zaman dilimi ve iskonto oranının sonuçlara etkisini degerlendirmek için tek yönlü duyarlılık analizi yapılmıstır. Bu amaçla %5, %7, %10 ve %15 iskonto oranları degerlendirilmis ve analizin iskonto oranına duyarlı olmadıgı bulunmustur. Arastırmanın kapsadıgı zamanın 10 yıl olarak degerlendirildiginde yine maliyet-etkililik sıralaması degismemis, yalnızca maliyet-etkililik oranları arasındaki büyüklük degismistir.
