Ekzotropya Hastalarında Cerrahi Sonuçlarımız

Abstract

Amaç: Anabilim Dalımız’da ekzotropyalı hastalara uyguladığımız şaşılık cerrahisi sonuç ve komplikasyonlarını değerlendirmek. Materyal ve Metod: Anabilim Dalımız’da 1997- 2007 yılları arasında şaşılık cerrahisi uygulanan 30 % 41,7 kadın, 42 % 58,3 erkek, toplam 72 ekzotropya hastasının kayıtları retrospektif olarak tarandı. Ortalama hasta yaşı 17,42 ± 7,84 olarak tespit edildi. Toplam 108 geriletme ve 31 rezeksiyon yapıldı. Hastalara yapılan ortalama müdahale miktarı 12,79 ± 3,82 mm olarak saptandı. Bulgular: Ortalama preoperatif ekzodeviasyon derecesi 41,71 ± 14,65 prizm diyoptri PD 10-80 PD olarak saptandı. Postoperatif dönemde hastalar ortalama 4,63 ± 8,15 ay 1 gün- 4 yıl takip edildiler. Postoperatif 1. hafta sonunda ortalama kayma derecesi 8,03 ± 11,58 PD 0-40 PD oldu. Hastalarda cerrahi sonrası düzelme miktarı 34,51 ± 15,46 PD 10-80 PD oldu. Her 1 mm müdahaleye karşılık gelen düzelme cerrahi etkinlik miktarı 2,93 ± 1,52 PD 0,67-8,75 PD olarak tespit edildi. Yaş ile cerrahinin etkinliği arasında güçlü, pozitif, doğrusal bağıntı saptandı rp= 0,317, p= 0,007 . Cerrahinin etkinliği ile preoperatif kayma derecesi arasında güçlü, pozitif, doğrusal bağıntı saptandı rp= 0,332, p= 0,004 . Ekzotropyalı hastalarda preoperatif kayma derecesi ile postoperatif son kayma dereceleri arasında bağıntı analizi yapıldığında, iki değişken arasında lineer, güçlü, pozitif bir ilişkinin varlığı tespit edildi rp= 0,322, p= 0,0006 . Postoperatif dönemde 3 % 4,17 hastada aşırı düzeltme saptandı. Sonuç: Yapılan regresyon analizinde şu formül tespit edildi: Müdahale miktarı mm = 9,45 – 0,1 × yaş + 0,12 × preoperatif kayma derecesi PD

Outcomes of strabismic surgery in exotropic patients

Abstract

Aim: To evaluate the outcomes and the complications of the strabismic surgeries performed in the exotropic patients at our Department of Ophthalmology. Methods: This retrospective study included 72 30 female, 42 male esotropic patients that underwent strabismic surgery at Yüzüncü Yıl University, Medical Faculty, Department of Ophthamology between 1997 and 2007. Mean patient age was 17.42 ± 7.84 range, 1 to 42 years . Surgical interventions included 108 recession and 31 resection procedures. Mean amount of surgical intervention was 12.79 ± 3.82 mm. Results: Mean preoperative degree of exodeviation was 41.71 ± 14.65 prism diopters PD range, 10 to 80 PD . Mean postoperative follow-up was 4.63 ± 8.15 months 1 day- 4 years . Mean degree of deviation 1 week postoperatively was 8.03 ± 11.58 PD range, 0 to 40 PD . Postoperatively, mean surgical alignment was 34.51 ± 15.46 PD range, 10 to 80 PD . The amount of alignment obtained per each 1 mm surgical intervention efficiency of surgery was 2.93 ± 1.52 PD range, 0.67-8.75 PD . A strong, positive, linear relation between age of patients and efficiency of surgery was detected rp= 0.317, p= 0.007 . A strong, positive, linear relation between preoperative degree of deviation and efficiency of surgery was detected rp= 0.332, p= 0.004 . Regression analysis performed between preoperative degree of deviation and latest postoperative degree of deviation in exotropic patients revealed a strong, positive, linear relation rp= 0.322, p= 0.0006 . Postoperatively 3 4,17% patients had overcorrection. Conclusion: Regression analysis performed revealed the following formula: Amount of surgical intervention mm = 9.45 – 0.1 × age + 0.12 × preoperative degree of deviation PD
