Esotropyada Cerrahi Sonuçlarımız

Abstract

Amaç: Kliniğimizde esotropya ET hastalarına uygulanan şaşılık cerrahisi sonuçlarını irdelemek. Gereç ve Yöntemler: Yüzüncü Yıl Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı’nda 1996-2007 yılları arasında ET nedeniyle opere edilen 73 kadın, 63 erkek, toplam 136 hasta retrospektif olarak değerlendirildi. Bulgular: Hastalara toplam 221 geriletme ve 73 rezeksiyon prosedürü uygulandı. Hastaların ortalama preoperatif kayma miktarı 45,18 ± 18,57 prizm diyoptri PD 15-90 PD idi. Hastalar postoperatif dönemde ortalama 6,21 ± 10,42 ay 1 gün-72 ay süreyle takip edildi. Yapılan ortalama toplam geriletme + rezeksiyon müdahale miktarı 12,34 ± 3,5 mm oldu. Ortalama düzelme miktarı 35,54 ± 20,82 PD olarak saptandı. Her 1 mm cerrahi için elde edilen düzelme 2,89 ± 1,45 PD olarak gerçekleşti. Olgu yaşı ile her 1 mm cerrahi için elde edilen düzelme miktarı arasında pozitif doğrusal güçlü bir bağıntı varlığı tespit edildi rp= 0,243, p= 0,004 . Preoperatif kayma derecesi ile her 1 mm cerrahi için elde edilen düzelme miktarı arasında pozitif doğrusal güçlü bir bağıntı varlığı tespit edildi rp= 0,579, p= 0,000 . Postoperatif son kontrolde 51 % 36,7 hastada ortoforya, 79 % 56,8 hastada ET, 9 % 6.5 hastada ise ekzotropya tespit edildi. Hastaların ortalama postoperatif son kayma miktarı 11,58 ± 11,19 PD 0-50 PD olarak tespit edildi. Sonuç: Uygulanacak toplam cerrahi miktarını tespit için uygulanan Regresyon analizinde şu formül saptandı: Yapılacak toplam cerrahi müdahale miktarı= 8,68 -0,03 × yaş + 0,091 × PD -0,056 × cinsiyet

Outcomes of Surgery in Esotropic Patients

Abstract

Objective: To evaluate the results of strabismic surgeries performed at our clinic. Material and Methods: This retrospective study included 136 73 female, 63 male esotropic patients that underwent strabismic surgery at Yüzüncü Yıl University Department of Ophthalmology between 1996 and 2007. Results: A total of 221 recession and 73 resection procedures were performed. Mean preoperative deviation degree was 45.18 ± 18.57 prism diopters PD range, 15-90 PD . The mean follow-up was 6.21 ± 10.42 months range 1 day-72 months postoperatively. The mean total recession + resection surgical intervention amount was 12.34 ± 3.5 mm. The mean correction degree was 35.54 ± 20.82 PD. The correction obtained per each 1 mm surgical intervention was 2.89 ± 1.45 PD. A strong, positive, linear relation between age of patient and correction obtained per each 1 mm surgical intervention was detected rp= 0.243, p= 0.004 . A strong, positive, linear relation between preoperative degree of deviation and correction obtained per each 1 mm surgical intervention was detected rp= 0.579, p= 0.000 . At last postoperative examination 1, 51 36.7% patients had orthophoria, 79 56,8% had esotropia, and 9 6.5% had exotropia. At the last postoperative examination the mean deviation degree in the patients was 11.58 ± 11.19 PD range, 0-50 PD . Conclusion: Regression analysis performed to find out the total surgical intervention amount to be performed revealed the following formula: The total surgical intervention amount to be performed = 8.68 -0.03 × age + 0.091 × PD -0.056 × sex
