BASI YARASINDA MAGGOT DEBRİDMAN TEDAVİSİ: OLGU SUNUMU

Abstract

Günümüzde bası yarası bilimsel ve teknolojik gelişmelerle birlikte önlenebilir bir komplikasyon olmasına karşın halen morbidite ve mortalitesiyle güncelliğini korumaya devam etmektedir. Bası yarası, uzun süreli ya da yineleyen basılara bağlı olarak özellikle kemik çıkıntılarının bulunduğu bölgelerde dolaşımın durması sonucu ortaya çıkan nekroz ve ülserayonlardır. Bası yaralarının oluşumunu önlemede en etkili yol neden olan faktörlerin belirlenmesi ve gerekli önlemelerin alımasıdır. Basınç yarası oluştuktan sonraki en önemli basamak ise yaranın uygun ve etkili yöntemler ile bakımıdır. Bası yarası için kullanılan konvensiyonel yara bakımına ve yara bakım ürünlerine cevap vermeyen enfekte kronik yaralarda ve tedavi yöntemlerinin ilerleyen doku kaybını engelleyemediği durumlarda Maggot debridman tedavisi(MDT) uygulanabilmektedir. Maggot debridman tedavisi; Lucilia Sericata adlı yeşil sineğin larvaları, yara üzerinde nekrotik ve enfekte dokuyu eriterek yarayı debride etmekte, yaranın dezenfeksiyonu ve oksijenasyonunu sağlamakta ve yara iyileşimini hızlandırmaktadır. Bu olgu sunumunda yatışının 45. gününde Evre 3 bası yarası gelişen hastanın konvensiyonel tedaviye direnç göstererek iyileşme sağlanamayan bası yaralarına uygulanan Maggot debridman tedavisi ve sonuçlarının sunulması amaçlanmıştır

MAGGOT DEBRIDEMENT THERAPY ON PRESSURE WOUND: CASE REPORT

Abstract

Today, the sores can be prevented with scientific and technological developments Although
still a complication to date with morbidity and mortality continues to protection. Pressure ulcer,
prolonged or repeated compression due to necrosis resulting from the cessation of circulation,
especially in areas where the bones protrude and are ülserayon. Causative factor in determining the
most effective way to prevent the formation of pressure sores and to take the necessary prevention.
The most important step after the injury occurs is pressure wound care with appropriate and
effective methods. Conventionally used for pressure ulcer wound care and wound care products to
answer the infected cases Maggot debridement therapy can not prevent progressive tissue loss and
treatment of chronic wounds (MDT) can be applied. Maggot debridement therapy; Lucilia sericata
larvae from the green fly, on necrotic and infected wounds by dissolving tissue to debride the
wound, the wound accelerates provide disinfection and oxygenation and wound healing. In this
case report 45 days of hospitalization in Phase3 pressure ulcer developing pressure sores can not
be applied to patients showing resistance to conventional therapy achieved improvement Maggot
debridement therapy and is aimed to present the results.
