Pitozis cerrahisinde farklı teknik ve materyaller

Abstract

Amaç: Bu çalışmada pitozis nedeniyle ameliyat edilen hastalarda farklı cerrahi teknik ve materyallerin sonuçları değerlendirildi.

Gereç ve yöntemler: Göztepe Eğitim ve Araştırma Hastanesi Göz Hastalıkları kliniğinde Ekim 2008 - Kasım 2009 tarihleri arasında pitozis tanısı ile 25 hastanın (15 erkek, 10 kadın; ort. yaş 38.8 yıl; dağılım, 3-78 yıl) 35 göz kapağı ameliyat edildi ve sonuçlar prospektif olarak incelendi.

Bulgular: Pitozis türü 10 hastada doğuştan (6 myojenik, 3  aponevrotik ve 1 nörojenik) ve 15 hastada edinsel (14 aponevrotik ve 1 nörojenik) idi. Aponevrotik pitozisli 17 hastaya levator aponevroz cerrahisi, zayıf levator fonksiyona sahip yedi hastaya frontal askılama cerrahisi ve nörojenik (Marcus Gunn jaw-winking) pitozisi olan bir hastaya levator rezeksiyonu ile kombine frontal askılama cerrahisi uygulandı. Yirmi iki hastada (%88) istenen kapak düzeyi elde edildi. Takip süresi ortalama 8.5 ay (3-13 ay) idi. Takip süresince 35 gözün ikisinde (%5.7) nüks gelişti.




Sonuç: Pitozis için uygulanacak cerrahi yönteme karar vermede klinik değerlendirme önemlidir. Patogeneze göre doğru endikasyon ile cerrahilerin sonuçları oldukça tatmin edicidir.
