The effect of neutrophil, lymphocyte, LDH and NLO on overall survival as a prognostic parameter in small cell lung cancer (SCLC)

Abstract

Aim: Small cell lung cancer (SCLC) is a rapidly progressing cancer with poor prognosis. In this study, we compared the laboratory results of patients with SCLC with control group and we wanted to show the relationship between the survival rates of neutrophil, LDH, NLR and lymphocytes. Materıal and Methods: The groups with the diagnosis of SCLC (n = 20) were compared on the basis of neutrophil, lymphocyte, NLR and LDH values. IBM SPSS 20 program was used for statistical analysis. Mann-whitney u test was used to compare parametrical data between independent groups. Kapplan Meier test was used for survival analysis. Survival difference between groups was compared with log rank test. Results: The LDH value of the patients was 340 (277-558) and the control group was 164 (136-181) (p <0.001).The neutrophil value of the patients was 5.3 (4.3-9.3) and the value of the control group was 3.7 (2.9-4.5) and the difference was statistically significant (p <0.001).The lymphocyte value of the patients was 1.1 (0.9-1.4) and this value was 2.4 (1.9-3.2) in the control group and it was statistically significant (p <0.001).The NLR of the patients was 4.3 (3.2-9.0) and the control group was 1.5 (1.1-2.0); this difference was statistically significant (p <0.001).The median LDH, neutrophil and NLL values of patients with SCLC were significantly higher than the control group; The median lymphocyte count was significantly lower in patients with SCLC (p <0.001). The median overall survival of patients with NLR 5.6 was not reached yet; The median overall survival of patients with NLR 5.6 and above was 6 months. The overall survival of patients with NLR 5.6 was significantly higher than patients with NLR 5.6 and above (p = 0.006). Conclusıon: Increased NLO in SCLC is associated with poor prognosis. When we compared the values of patients with SCLC with Control Group, we showed that decreased lymphocyte value with decreased neutrophil, LDH and NLR was associated with decreased survival.

Küçük hücreli akciğer kanserinde (KHAK) prognostik parametre olarak nötrofil, lenfosit, LDH ve NLO değerleri ile NLO’nun sağkalıma etkisi

Öz

Amaç: Küçük hücreli akciğer kanseri (KHAK) hızla progrese olan ve kötü prognoza sahip bir kanserdir. Bu çalışmamızda KHAK’li hastalarımızın değerlerini Kontrol Grubu ile kıyaslayarak artmış nötrofil, LDH ve NLO ile azalmış lenfosit değeri kısalmış sağ kalımla birlikte olduğunu göstermek istedik. Gereç ve Yöntemler: KHAK tanısı olan grup (n=20) ile kanser tanısı olmayan sağlıklı grup (n=20) nötrofil, lenfosit, NLO ve LDH değerleri bazında karşılaştırıldı. İstatistiksel analiz için IBM SPSS 20 programı kullanıldı. Bağımsız gruplar arasında parametrik verilerin karşılaştırılmasında mann-whitney u testi kullanıldı. Sağ kalım analizleri için Kapplan Meier testi kullanıldı. Gruplar arasında sağ kalım farkı log rank testi ile kıyaslandı. Bulgular: Hastaların LDH değeri 340 (277-558), Kontrol Grubunun ise 164 (136-181) idi (p <0.001).Hastaların nötrofil değeri 5.3 (4.3-9.3) iken Kontrol Grubunun değeri 3.7 (2.9-4.5) idi aradaki fark istatistiksel olarak anlamlı idi (p<0.001). Hastaların lenfosit değeri 1.1 (0.9-1.4) iken Kontrol Grubunda bu değer 2.4 (1.9-3.2) bulundu ve istatistiksel olarak anlamlı idi (p<0.001). Hastaların NLO oranı 4.3 (3.2-9.0) ve Kontrol Grubunun 1.5 (1.1-2.0) idi; bu fark istatistiksel olarak anlamlı idi (p<0.001). KHAK’li hastaların ortanca LDH, nötrofil ve NLO değerleri kontrol grubundan belirgin olarak yüksek; ortanca lenfosit değeri ise KHAK'li hastalarda belirgin olarak düşük saptandı (p<0,001). NLO <5,6 olan hastaların ortanca genel sağ kalımına henüz ulaşılmadı; NLO değeri 5,6 ve üzeri olan hastaların ortanca genel sağ kalımı 6 ay idi. NLO <5,6 olan hastaların genel sağ kalımı, NLO 5,6 ve üzeri olan hastalardan belirgin olarak yüksek saptandı (p=0,006). Sonuç: KHAK’de artmış NLO kötü prognozla birliktedir. KHAK’li hastalarımızın değerlerini Kontrol Grubu ile kıyasladığımızda, artmış nötrofil, LDH ve NLO ile azalmış lenfosit değerinin azalmış sağ kalımla birlikte olduğunu gösterdik.
