Total Diz Artroplastisi Öncesi ve Sonrası Denge ve Düşme Riskinin  Değerlendirilmesi

  • Amaç:Bu çalışmanın amacı diz osteoartritli hastaların total diz artroplastisi (TDA) öncesi ve sonrası denge ve düşme risklerini incelemektir. Gereç ve Yöntem: İzmir Kâtip Çelebi Üniversitesi Atatürk Eğitim ve Araştırma Hastanesi Ortopedi ve Travmatoloji Servisi’nde primer TDA uygulanan 46 hastanın demografik bilgileri Şubat 2017 ile Mayıs 2018 tarihleri arasında kayıt edildi. Süreli kalk yürü testi, ikili görevli süreli kalk yürü testi, kısa Balance Evaluation-Systems Test (BESTest) ve The Fall Risk for Older People in the Community (FROP-Com) (Toplum Koşullarında Yaşayan Yaşlı Bireyler için Düşme Riski) anketi ile denge ve düşme riskleri değerlendirildi. Bulgular:Çalışmada yaş ortalaması 67.96±1.01 yıl olan 46 (36 kadın, 10 erkek) hastanın verileri incelendi. Süreli kalk yürü testi, süreli kalk yürü testi motor görev, süreli kalk yürü testi bilişsel görev sürelerinin TDA sonrasında TDA öncesine göre anlamlı olarak düşük olduğu bulunmuştur (p˂.05). Kısa-BES Test değerlendirme sonuçlarının TDA sonrasında TDA öncesine göre anlamlı olarak yüksek olduğu bulunmuştur (p˂.05). TDA öncesi ve sonrası FROP-Com sonuçları ve düşme oranları arasında anlamlı fark bulunmamıştır (p˃.05). Sonuç:Diz osteoartritli hastaların TDA sonrasında dengelerinin geliştiği ve düşme risklerinin azaldığı görülmüştür. TDA öncesi düşme oranı %23.9 iken, TDA sonrası ilk üç ayda %17.4 bulunmuştur.

Objective:To determine balance and risk of fall of patients with knee osteoarthritis before and after total knee arthroplasty (TKA). Material and Methods:Demographic characteristics of 46 patients who were applied primary TKA at Department of Orthopedics and Traumatology at Izmir Kâtip Çelebi University Atatürk Training and Research Hospital were recordedbetween the dates of February 2017 - May 2018. Balance and risk of fall were evaluated by Time Up and Go Test, Time Up and Go Dual Task Tests, The Brief Balance Evaluation-Systems Test (BESTest), The Fall Risk for Older People in the Community (FROP-Com) questionnaire. Findings:Data of 46 patients with knee osteoarthritis (36 females and 10 males, mean age=67.96±1.01years) were examined in the study. The duration of Time Up and Go Test, Time Up and Go Test Motor Task, Time Up and Go Test Cognitive Task were significantly lower after TKA compared to before TKA (p˂.05). The results of The Brief-BESTest evaluation were significantly higher after TKA compared to before TKA (p˂.05). There were no significant differences between FROP-Com results and fall rate before and after TKA. Conclusion:Patients with knee osteoarthritis have been found to develope their balance and decrease the risk of fall after TKA. While the fall rate was 23.9% before TKA, it was 17.4% in the first three months after TKA.
