RANİTİDİN BİZMUT SİTRAT VE KLARİTROMİSİNLİ İKİLİ KOMBİNASYONLA ALTERNATİF HELİKOBAKTER PYLORİ TEDAVİSİ

Abstract

Amaç: Endoskopik incelemesinde Helikobakter pylori (Hp) enfeksiyonunun eşlik ettiği duodenal ülser ve/veya gastrit tesbit edilen 44 hastada Ranitidin bizmut sitrat (RBS) ve Klaritromisin kombinasyonunun tedavide etkinliğini araştırmak. Gereç ve yöntem: Nisan 1998, Mayıs 1999 tarihleri arasında gastroskopilerinde Hp enfeksiyonunun eşlik ettiği duodenal ülser ve/veya gastrit tesbit edilen, çalışmaya alınmayı kabul eden, ardışık 44 hasta prospektif olarak çalışmaya alındı. Hastalara 30 gün süreyle RBS 400 mg 2x1, 14 gün süreyle klaritromisin 500 mg 2x1 verildi, tedavi bitiminden 1 ay sonra kontrol endoskopik incelemeleri yapıldı. Bulgular: 44 hastanın 15’i erkek, 29’u kadın; yaş ortalaması 45,93±14,03 dağılımı 17-85 idi. En sık gözlenen semptomlar epigastrik ağrı (%100), pirozis ve şişkinlik idi. Tedavi sonrası hastaların şikayetlerinde belirgin gerileme gözlendi. Onüç duodenal ülserli hastadan 12’si iyileşti (% 92,3) ve 44 hastadan 37’sinde (% 84,09) Hp eradikasyonu sağlandı. Sonuç: RBS ve klaritromisin kombinasyonuyla %84,09 oranında Hp eradikasyonu sağlanmış olup bu kombinasyonun klasik üçlü eradikasyon tedavisine özellikle dirençli bölgelerde alternatif olabileceği düşünüldü.
