EPİLASYON AMACIYLA PRİLOKAİN UYGULANAN BİR HASTADA GELİŞEN METHEMOGLOBİNEMİ

Abstract

Normal şartlarda hemoglobinde bulunan demir iki değerlidir (Fe++). Hemoglobindeki demirin okside olup üç değerli (Fe+++) duruma geçmesiyle methemoglobinemi oluşur. Bu reaksiyon, dokuda hipoksemiye yol açar. Ağır vakalarda hemoglobinin, oksijen ve karbondioksit taşıma kapasitesindeki ciddi bozulma nedeniyle ölüm meydana gelebilir. Methemoglobinemi sıklıkla kimyasal maddelerle temas sonrası görülmekle beraber, genetik, besinlere bağlı ve idiopatik olarak da bulunabilir. Lokal anestetiklere bağlı methemoglobinemi daha çok yenidoğanlarda karşılaşılan bir durumdur. Bu yazıda epilasyon işlemi öncesi prilokain uygulanan ve sonrasında akut methemoglobinemi gelişen erişkin bir olgu sunulmuştur.
