UZUN DÖNEM PERKÜTAN NEFROLİTOTOMİ PNL SONUÇLARIMIZ: 4 YILLIK KLİNİK DENEYİMİMİZ

Öz

Bu çalışmada kliniğimizde son 4 yıl içerisinde gerçekleştirilen ve hasta kayıtlarına ulaşılabilen perkütan nefrolitotomi PNL sonuçlarımızın değerlendirilmesi amaçlanmıştır. Çalışmaya kliniğimizde 2010 ve 2014 yılları arasında böbrek taşı tanısıyla PNL operasyonu yapılan ve hasta kayıtlarına ulaşılabilen 409 hasta dahil edilmiştir. Hastalardan operasyon öncesi idrar kültürü çalışılarak üremesi olanlara antibiyotik tedavisi altında veya sonrasında PNL yapıldı. Tüm hastalara standart olarak litotomi pozisyonunda skopi eşliğinde üreteral kateter yerleştirilmesi sonrasında prone pozisyon verilerek floroskopi eşliğinde perkütan giriş yapılmıştır. Amplatz dilatatörler kullanılarak ve sonrasında 30 f amplatz sheath yerleştirilerek operasyonlar gerçekleştirildi. Taş fragmentasyonu tüm hastalarda pnömotik litotriptör yardımı ile yapıldı. Tüm hastalarda postoperatif 1. gün direkt üriner sistem grafisi DÜSG veya üriner sistem bilgisayarlı tomografi BT ile değerlendirme yapıldı. Taşların tamamen temizlenmesi veya ≤4 mm rezidüel taş kalması başarı olarak kabul edildi. Hastaların demografik özellikleri, perioperatif özellikler, postoperatif sonuçlar ve komplikasyonlar edildi. Hastaların 274’ü erkek ve 135’i ise kadındı. Ortalama taş yükü 5,72 cm2 1,02-28,26 olarak hesaplandı. not edildi. Hastaların yaş ortalaması 48,4 olarak tespit Postoperatif başarı oranı %74,8 olarak hesaplandı. 409 hastada toplam 148 komplikasyon izlendi. Modifiye clavien sınıflamasına göre değerlendirme yapıldığında 72 hastada evre 1 %17,6 , 59 hastada evre 2 %14,4 , 6 hastada evre 3a %1,4 ve 11 hastada evre 3b %2.6 komplikasyon izlendi. Hiçbir hastada evre 4 veya 5 komplikasyon izlenmedi. Postoperatif takipte en sık izlenen komplikasyonlar nefrostomi yerinden ıslatma, ağrı ve transfüzyon gerektirmeyen kanama idi. Komplike böbrek taşı olan hastalarda komplikasyon oranlarının anlamlı olarak daha fazla olduğu tespit edildi. Toplam olarak 26 hastaya %6,3 taşına yönelik ek tedavi PNL, URS-L, ESWL vb. uygulandı. PNL böbrek taşlarının cerrahi tedavisinde kabul edilebilir komplikasyon oranları ile etkili ve güvenilir bir yöntemdir. Özellikle taş lokalizasyonu ve taş yükü başarı oranını en çok etkileyen faktörlerdendir

LONG-TERM PERCUTANEOUS NEPHROLITHOTOMY PNL OUTCOMES: 4 YEAR FOLLOW-UP

Abstract

In this study we aimed to evaluate the results of percutaneous nephrolithotomy PNL which performed in our clinic in the last 4 years with accessible patients’ records. Four hundred nine patients who had kidney stones and underwent PNL between 2010-2014 were included in this study. Preoperative urine culture were performed. PNL procedures were performed under antibiotic treatment or after necessary treatment if urine culture was positive. Percutaneous access was performed in all of the patients in prone position with fluoroscopy following ureteral catheter placement in the lithotomy position with scopy. Operations were carried out using Amplatz dilators and a 30 F Amplatz sheath was placed. Stone fragmentation was performed in all of the patients with pneumatic lithotripter. All patients were evaluated by direct urinary system graphy DUSG or urinary tract computed tomography CT on the postoperative first day. No residual stones or ≤4 mm residual stone was considered as success. Demographic characteristics of the patients, perioperative characteristics, postoperative results and complications were noted. The mean age of the patients was 48.4 . 274 patients were male and 135 were female. The mean stone burden was 5.72 cm2 1.02 to 28.26 . Mean operation time was 110.4 minutes and fluoroscopy time was 95 seconds . The mean length of hospital stay was 4.06 days , and the nephrostomy time was 2.3 days . 39 patients 9.5% had blood transfusion in the postoperative period. Postoperative success rate was calculated as 74.8%. It was a total of 148 complications in 409 patients. Evaluating by the modified clavien classification, 72 patients were stage 1 17.6% , 59 patients were stage 2 14.4% , 6 patients were stage 3 1.4% , 11 patients were stage 3b 2.6% . No patients had stage 4 or 5 complication. The most common complication in the postoperative follow-up was leakage from nephrostomy area, bleeding which didn’t require transfusion and pain. Patients with complicated kidney stone had significantly greater rates of complications. 26 patients 6.3% underwent additional treatment for residual stone PNL, URS-L, ESWL, etc. PNL is an acceptable surgical treatment for kidney stones. Especially, stone location and stone burden are critical factors in the success rate
