EFFICIENCY OF URETEROSCOPY AFTER FAILED SWL IN MID AND UPPER URETERAL STONES

Abstract

Introduction: Technological development in ureteroscopic instruments and the usage in situ stone fragmentation devices, have increased the success rates of endoscopic treatment in middle and upper ureteral stones. Today, the vast majority of SWL failed or contraindicated of urinary tract stone disease are treated with endourologic modalities. In this study, we report the results of ureteroscopic procedures in middle and upper ureteral stones whose SWLtreatment were failed. Material and method: Between February 2002 and May 2004, we performed ureteroscopic procedures to mid and upper ureter stones to 72 patients 46 male, 26 female who had SWL failed. Ureteroscopic lithotripsy was performed by pneumatic lithotripters. The mean age value was 43.75 years. Results: Stones were examined in two groups as upper and mid ureter localizations. Ureteroscopy was performed to 31 43% patients in left ureter and 41 57% patients in right ureter stones. The average stone size was 0.92 0.31.8 cm. Mean operation time was 26.8 minutes and overall success rate was 87.5%. Over all complication rates are 7% in mid ureter group and 21% in upper ureter group. Conclusions: If there is no severe obstruction related with stone and stone is non-transparent, first line treatment in upper ureteral stones must be SWL. If not, it will be more rationalistic to consider ureteroscopic procedures. In midureteral stone localization, the first line treatment should be ureteroscopic procedures because of low SWL success rate and the need for anesthesia during SWL treatment

ESWL’YE DİRENÇLİ ORTA VE ÜST BÖLÜM ÜRETER TAŞLARINDA ÜRETERORENOSKOPİNİN ETKİNLİĞİ

Abstract

Giriş: Teknolojik gelişmeler sayesinde üreteroskopik aletlerdeki değişmeler ve in situ taş kırmaya yarayan cihazların kullanıma girmesi ile orta ve üst üreter taşlarının tedavisinde de endoskopik tedavilerin başarı oranları giderek artmıştır. Günümüzde ESWL’ nin başarısız ya da sakıncalı olduğu üriner sistem taş hastalığının büyük çoğunluğu endoürolojik yöntemlerle tedavi edilmektedir. Bu çalışmada, ESWL tedavisi başarısız olan orta ve üst üreter taşlarında üreteroskopik girişimin sonuçlarını sunmaktayız. Gereç ve Yöntem: ESWL tedavisi başarısız olan 72 hastaya 46 erkek, 26 kadın Şubat 2002 - Mayıs 2004 tarihleri arasında kliniğimizde üreter orta ve üst bölüm taşlarına üreteroskopik girişim uygulanmıştır. Üreteroskopik litotripsi pnömatik litotriptör ile uygulandı. Hastaların yaş ortalaması 43.75 yıldır. Bulgular: Taşlar lokalizasyonuna göre, üst bölümde yerleşim gösteren ve orta bölümde yerleşim gösteren olmak üzere iki grupta incelendi. Otuz bir %43 hastada sol üreter, 41 %57 hastada ise sağ üreter taşlarına üreteroskopi uygulanmıştır. Ortalama taş çapı 0.92 0.3-1.8 cm olarak bulunmuştur. Ortalama operasyon süresi 26.8 dakika ve total başarı oranımız % 87.5 olarak bulunmuştur. Üreter orta bölüm lokalizasyonlu taşlarda komplikasyon oranımız %7, üreter üst bölüm lokalizasyonlu taşlarda ise %21’dir Sonuç: Üreter üst uç taşlarında, taşa bağlı ileri derecede obstrüksiyon yoksa ve taş opaksa ilk tedavi seçeneği olarak ESWL düşünülmelidir. Aksi takdirde üreteroskopik tedavilerin denenmesi daha akılcı olabilir. Taş orta üreter lokalizasyonunda ise ESWL tedavisinin başarısı daha düşük oranda olması ve ESWL sırasında sıklıkla anestezi gerekliliği nedeniyle ilk basamak tedavisi üreteroskopik yöntemler olmalıdır
