Nadir Pediatrik Olgu: Nazal Miyazis

Abstract

Amaç: ‘İnsan miyazisi’ çoğunlukla Sarcophagidae ailesine ait sineklerin larvaları veya kurtçuk formları ile farklı doku tutulumlarına sebep olan zoonotik infeksiyondur. Genellikle keçi ve koyunların burun delikleri ve çevre dokularına yerleşerek miyazise sebep olan Oestrus ovis insanlarda da nadir de olsa nazal ve paranazal bölgede izlenebilmektedir. Olgu: Son bir aydır burun tıkanıklığı şikayeti olan yedi yaşındaki erkek hastamıza nazal endoskopi, yüz ve nazofarengeal MR görüntüleme sonrasında sağ nazal kavitesinde şüpheli yabancı cisim tanısı koyuldu. Dekonjestan ve serum fizyolojik ile nazal irrigasyon sonrasında canlı larvalar temizlendi. Mikroskopik değerlendirmede ölçüsü 10 mm larvaların Oestrus ovis larvaları olduğu bildirildi. Bir yıllık kontrolünde nazofarinks olağan olarak değerlendirildi. Sonuç: Pediatrik yaş grup çocukların burun ve paranazal sinüslerinde Oestrus ovis miyazisi nadirdir. Burun tıkanıklığı ile başvuran çocuklarda, ’Oestrus ovis’’ nazal miyazisinin de göz önünde bulundurulması gerektiğini düşünmekteyiz.

A Rare Pediatric Case : Nasal Myiasis

Abstract

Aim: Human myiasis is a zoonotic infection in which various tissue involvements of larvae or maggot forms of fly species belonging to the Sarcophagidae family are often observed. Oestrus ovis, which mainly causes myiasis by obligatory placement in the nostrils of sheep and goats, can be aslo observed in the nasal and paranasal regions of humans, although it is rare. Case: A 7-year-old male patient with a complaint of nasal congestion for the past one month was diagnosed with a suspicious foreign body in the right nasal cavity after nasal endoscopy, facial and nasopharyngeal MRI imaging. After nasal irrigation with decongestant and serum physiology, live larvae were cleared. Microscopic evaluation of the larvae with a size of 10 mm was reported as Oestrus ovis larvae. The nasopharynx was evaluated as normal in the 1-year follow-up. Conclusion: Oestrus ovis myiasis in the nasal and paranasal sinuses of pediatric children is rare. We think that 'Oestrus ovis' nasal myiasis should be considered in children presenting with nasal obstruction.
