Ani Gelişen Nontravmatik Obstruktif Retrofarengeal Hematom : Nadir Bir Olgu

Abstract

Amaç: Retrofarengeal hematom gelişimi, çok nadir izlenen fakat yaşamı tehdit eden ani hava yolu obstrüksiyonu tehlikesine yol açabilen ve acil hava yolu stabilizasyonu gerektiren Kulak Burun Boğaz acil durumlarındandır. Bu nadir olgu sunumunun amacı hayatı tehdit eden bu gibi durumlara yönelik farkındalığı artırmak ve literatüre katkıda bulunmaktır. Olgu: Burada hastanemizin acil bölümüne son 4 saatte aniden başlayan ve progresif ilerleyen solunum sıkıntısı, disfaji, disfoni şikayetleri ile baş vuran, ileri derece epiglot ödemi ve retrofarengeal hematoma bağlı totale yakın larengeal obstruksiyon gelişen nadir bir olgu sunulmaktadır. Sonuç: Bu yazı, düzensiz varfarin ve aspirin kullanan, yağdan fakir beslenen ve karaciğer fonksiyon bozukluğu sonucu K vitamin eksikliği oluşan bir olguda retrofaringeal hematom gelişebildiğini gösteren literatürde bildirilen (PubMed, ScienceDirect, Scopus ve Google Scholar veri tabanları) ilk vaka örneğidir.

Sudden Nontraumatic Obstructive Retropharyngeal Hematoma: A Rare Case

Abstract

Aim: The retropharyngeal hematoma is an ear nose and throat emergency that is very rare but can cause the danger of sudden airway obstruction, which is life-threatening and requires urgent airway stabilization. This rare case report aims to increase awareness of such life-threatening situations and contribute to the literature. Case: In this report, I would like to present a rare case of severe epiglottic edema and near total laryngeal obstruction due to retropharyngeal hematoma, which contributed to the emergency department in our hospital with complaints of distress, dysphagia, and dysphonia that started suddenly in the last 4 hours. Conclusion: It is the first case example reported in the literature (PubMed, ScienceDirect, Scopus, and Google Scholar databases) showing that a person who uses irregular warfarin and aspirin, has a low-fat diet, and has vitamin K deficiency as a result of liver dysfunction may develop a retropharyngeal hematoma.
