Timpanoplasti ve Timpanomastoidektomi; 5 Yıllık Sonuçlarımız

Abstract
Amaç: Araştırmanın amacı kliniğimizde kronik otitis media (KOM) nedeniyle timpanoplasti ve timpanomastoidektomi uygulanan
hastalarda cerrahi sonuçlarımızı greft başarısı ve odyolojik açıdan güncel literatüre göre değerlendirmektir.
Gereç ve Yöntemler: Kulak Burun Boğaz Kliniğimizde Mayıs 2018- Mayıs 2023 tarihleri arasında KOM nedeniyle genel anestezi
altında timpanoplasti ve timpanomastoidektomi uygulanan 62 hasta çalışmaya dahil edilmiştir. Hastaların preoperatif anamnez,
fizik muayene ve odyolojik değerlendirmeleriyle; postoperatif süreçteki işitme testleri ve zar muayeneleri retrospektif olarak yaş ve
cinsiyet değişkenleri dahil edilerek istatistiki olarak değerlendirilmiştir.
Bulgular: Hastaların ortalama takip süresi 9.9 aydır. Preoperatif ve postoperatif hava kemik aralığı (HKA) değerlendirmesinde postoperatif işitme sonuçlarında istatistiksel olarak anlamlı iyileşme saptanmıştır (p<0.001). Greft başarı oranımız %91.9 bulunmuştur.
Hastaların %16’sında işitmede düzelme saptanmamıştır. %69.3’ünde ise %20’den daha yüksek oranda HKA kazancı saptanmıştır.
Yaşa göre greft başarısı ve odyolojik değerlendirmede istatistiksel farklılık saptanmamıştır. Kadın cinsiyette HKA kazancı erkek
cinsiyete göre daha yüksek bulunmuştur (p=0.034).
Sonuç: Timpanoplasti kronik otit hastalarında timpan membran perforasyonlarının onarımı, komplikasyonların önüne geçilmesi,
işitmenin rekonstrükte edilebilmesi açısından başarılı ve sıklıkla uygulanan bir cerrahidir

Abstract
Aim: The study aims to evaluate our surgical results in terms of graft success and audiology in patients who underwent tympanoplasty
and tympanomastoidectomy due to chronic otitis media (COM) in our clinic, according to the current literature.
Material and Methods: 62 patients who underwent tympanoplasty and tympanomastoidectomy under general anesthesia due to
COM in our Ear Nose and Throat Clinic between May 2018 and May 2023 were included in the study. With the preoperative
anamnesis, physical examination, and audiological evaluations of the patients; Hearing tests and membrane examinations in the
postoperative period were evaluated retrospectively and statistically by including age and gender variables.
Results: The average follow-up period of the patients was 9.9 months. A statistically significant improvement in postoperative
hearing results was detected in the preoperative and postoperative air bone gap (ABG) evaluation (p<0.001). Our graft success rate
was found to be 91.9%. No improvement in hearing was detected in 16% of the patients. ABG gain greater than 20% was detected
in 69.3% of the patients. No statistical difference was detected in graft success and audiological evaluation according to age. ABG
gain was found to be higher in the female gender than in the male gender (p=0.034).
Conclusion: Tympanoplasty is a successful and frequently performed surgery in chronic otitis patients in terms of repairing tympanic
membrane perforations, preventing complications, and reconstructing hearing.
