Ürogenital Kondilom Lezyonu Olan Erkek Hastalarda Lezyonda ve Üretral Sürüntü Örneğinde İPV Tanısı ve Tiplendirmesi

Abstract

Amaç: Ürogenital kondilomu olan erkek hastalarda İnsan Papilloma Virus (İPV) enfeksiyon varlığının tespitinde üretral sürüntü örneklerinin etkinliğini değerlendirmek ve mevcut olan bu enfeksiyonlarının sık görülen onkolojik tip olan İPV–16 ile olan ilişkisini ortaya koymak amaçlanmıştır.
Materyal-Metot: Kliniğimize ürogenital siğil şikâyeti ile başvuran cinsel olarak aktif, immunsupresif tedavi veya kemoterapi öyküsü olmayan erkek hastalar dahil edildi. Eksternal meatusda tespit edilen lezyonlar eksize edildi. Üretroskopi yapılarak üretrada başka lezyon olmadığı gösterildi. Kutanöz lezyonların belirgin olanları dokuda eksize edildi. Küçük lezyonlar elektrokoter ile koterize edildi. Tüm hastalardan viral tanı ve tiplendirme amacıyla üretral sürüntü örneği alındı. Elde edilen dokular virolojik çalışma ve patolojik incelemeye gönderilmek üzere ikiye ayrıldı. İPV tanısı için dokuda ve sürüntü örneklerinde polimeraz zincir reaksiyonu uygulandı.
Bulgular: Ortalama yaşı 34,9±9 yıl olan 34 hasta çalışmaya dahil edildi. 3 hastada eksternal meatusda verrüköz leyzon tespit edildi. 30 hastada patolojik inceleme sonucunda kondilom tespit edildi.1 hastada eşlik eden karsinoma in situ mevcuttu. 4 hastada ise seboreikkeratoz (2) ve akantoz (2) tespit edildi. Dokulardan yapılan virolojik çalışmalarda 18 hastada İPV varlığı gösterildi. İPV 16 mikst enfeskiyon gözlenen 6 örnekte gösterildi. İPV-16 varlığı malign ve benign lezyonlarda gösterildi (karsinoma in situ 1 hasta, akantoz 1 hasta). Üretral sürüntü örneklerinde ise 6 hastada İPV varlığı gösterildi. Üretral sürüntüde İPV (+) olan 2 hastanın doku örneğinde İPV(-)'ti.
Sonuç: İPV tanısında ve tiplendirmede üretral sürüntü örnekleri doku örnekleri kadar yeterli görünmemektedir.
