Abstract

Objective: In our study, we aimed to compare effects of preoperative dexketoprofen and tramadol administered by intravenous route on intraoperative and postoperative analgesic consumption, postoperative pain, durations of hospital stay and patient satisfaction in patients, undergoing laparoscopic cholecystectomy. Methods: After approval of ethic committee and written consent of patients were obtained, 60 patients between 18-70 years old with ASA I-II were included in the study. After routine monitorization and 20 minutes before induction of anesthesia, dexketoprofen 50 mg in 100 cc 0.9% NaCl was administered in Dexketoprofen Group and tramadol 100 mg in 100 cc 0.9% NaCl in Tramadol Group as intravenous infusion during 20 minutes. Intraoperative hemodynamic parameters, analgesic-anesthetic consumptions and complications of patients, on whom standard general anesthesia was applied, were recorded. Pain severity, degree of sedation, morphine consumptions and hemodynamic parameters were recorded at postoperative 30th minute, and 4th, 8th, 12th and 24th hours. Additional analgesia requirement,times for requiring first analgesia, duration of hospital stay, postoperative complications and patient satisfactions were recorded.Results: Intraoperative analgesic-anesthetic consumptions, postoperative visual analogue scale (VAS) scores, sedation degrees, intravenous patient controlled analgesia (PCA) and morphine consumptions, times for requiring first analgesic, durations of hospital stay, intraoperative-postoperative complications developed and patient satisfactions were similar in both groups. Consequently, postoperative analgesic efficacy of pre-emptive dexketoprofen intravenous 50 mg and tramadol intravenous 100 mg administered was found to be similar in cases, who had laparoscopic cholecystectomy operation.Conclusion: Because VAS scores were low in our patients, morphine consumptions with intravenous PCA were similar in both groups, and there wasn’t any additional analgesic requirement in our patients, we think that use of both tramadol and dexketoprofen for preemptive analgesia would be useful. Laparoskopik kolesistektomi uygulanan hastalarda preemptif deksketoprofen ve tramadol’ün postoperatif ağrı üzerine olan etkilerinin karşılaştırılması

Öz

Amaç: Çalışmamızda; laparoskopik kolesistektomi planlanan hastalarda, preoperatif intravenöz yoldan uygulanan deksketoprofen ile tramadolün intraoperatif ve postoperatif analjezik tüketimi, postoperatif ağrı, hastanede kalış süresi ve hasta memnuniyeti üzerine etkilerini karşılaştırmayı amaçladık. Yöntemler: Etik kurulu onayı ve hastaların yazılı onamları alındıktan sonra ASA I-II, 18-70 yaş arası, 60 hasta çalış- maya dahil edildi. Rutin monitorizasyon sonrası, anestezi indüksiyonundan 20 dk önce, Grup Deksketoprofen’de 100 cc %0,9 NaCl içinde deksketoprofen 50 mg, Grup Tramadol’de ise %0,9 NaCl içinde tramadol 100 mg intravenöz infüzyon şeklinde 20 dk’da uygulandı. Standart genel anestezi uygulanan hastaların intraoperatif hemodinamik parametreleri, analjezik-anestezik tüketimleri ve komplikasyonları kaydedildi. Postoperatif 30. dk, ve 4., 8., 12. ve 24. saatlerde ağrının şiddeti, sedasyon derecesi, morfin tüketimleri, hemodinamik parametreler kaydedildi. Ek analjezik gereksinimi, ilk analjezik gereksinim zamanları, hastanede kalış süreleri, postoperatif komplikasyonlar ve hasta memnuniyetleri kaydedildi. Bulgular: İntraoperatif analjezik-anestezik tüketimleri, postoperatif görsel ağrı skoru(VAS), sedasyon dereceleri, intravenöz Hasta Kontrollü Analjezi (HKA) ile morfin tüketimleri, ilk analjezik gereksinim zamanları, hastanede kalış süreleri, intraoperatif-postoperatif gelişen komplikasyonlar ve hasta memnuniyetleri her iki grupta benzerdi. Sonuç olarak, laparoskopik kolesistektomi operasyonu geçiren olgularda preemptif olarak uygulanan deksketoprofen intravenöz 50 mg ve tramadol intravenöz 100 mg’ın postoperatif analjezik etkinlikleri benzer bulundu. Sonuç: Hastalarımızdaki postoperatif VAS skorlarının düşük olması, intravenöz HKA ile morfin tüketimlerinin iki grupta da benzer olması ve ek analjezik gereksiniminin olmaması nedeniyle hem tramadolün hem de deksketoprofenin preemptif analjezi amacıyla kullanılmasının yararlı olacağı kanısındayız.
