Ankilozan spondilitli bireylerde servikal bölge eklem pozisyon hissi ve servikal bölge kaslarının mekanik özellikleri: kesitsel tanımlayıcı araştırma

Abstract

Amaç: Bu çalışmanın amacı, ankilozan spondilitli bireylerde servikal eklem pozisyon hissi ve servikal kasların mekanik özelliklerini sağlıklı bireylerle karşılaştırmaktır. Yöntem: Çalışmaya 30 ankilozan spondilitli ve 32 sağlıklı birey dahil edildi. Servikal eklem pozisyon hissi Cervical Range Of Motion (C-ROM) cihazı ile, kasların mekanik özellikleri Myoton Pro cihazı ile ve boyun ağrısı Northwick Park Boyun Ağrısı Anketi (NPBAA) ile değerlendirildi. Bulgular: Sol üst trapez kasının tonusu ve sertliği ankilozan spondilit grubunda sağlıklı bireylere göre anlamlı düzeyde daha yüksekti (p=0,027; p=0,032). Servikal ekstansiyon, lateral fleksiyon ve rotasyon hareket açıklıkları ankilozan spondilit grubunda anlamlı şekilde daha düşük bulundu (p<0,001; p=0,002). Boyun ağrısı skoru da ankilozan spondilit grubunda sağlıklı gruba göre anlamlı düzeyde yüksekti (p=0,001). Sonuç: Ankilozan spondilitli bireylerde üst trapez kasında tonus ve sertlik artışı ile birlikte servikal ekstansiyon, lateral fleksiyon ve rotasyon hareket açıklıklarında kısıtlılıklar gözlendi. Fleksiyon hareketinin görece korunmuş olması, postüral adaptasyon ve günlük kullanım alışkanlıklarıyla ilişkilendirilebilir. Kas mekanik özelliklerinin değerlendirilmesi, ankilozan spondilitin fonksiyonel etkilerini anlamada ve uygun rehabilitasyon planlamasında önemli bir parametre olarak öne çıkmaktadır.

Cervical joint position sensation and mechanical properties of cervical muscles in individuals with ankylosing spondylitis: A cross-sectional descriptive study

Abstract

Purpose: The aim of this study was to compare cervical joint position sense and the mechanical characteristics of cervical muscles in individuals with ankylosing spondylitis and healthy individuals. Methods: A total of 30 individuals with ankylosing spondylitis and 32 healthy individuals were included. Cervical joint position sense was evaluated using the Cervical Range Of Motion (C-ROM) device, mechanical properties of the muscles were measured with the Myoton Pro device, and neck pain was assessed using the Northwick Park Neck Pain Questionnaire (NPBAA). Results: Tonus and stiffness of the left upper trapezius muscle were significantly higher in the ankylosing spondylitis group compared to healthy controls (p=0.027; p=0.032). Cervical extension, lateral flexion, and rotation ranges of motion were significantly lower in the ankylosing spondylitis group (p<0.001; p=0.002). Neck pain scores were also significantly higher in the ankylosing spondylitis group (p=0.001). Conclusion: Increased tone and stiffness in the upper trapezius muscle, along with limitations in cervical range of motion cervical extension, lateral flexion, and rotation, were observed in individuals with ankylosing spondylitis. Preservation of cervical flexion may be related to postural adaptation and habitual daily use. Evaluating muscle mechanical properties is important for understanding functional effects of ankylosing spondylitis and planning appropriate rehabilitation.
