Farklı inseminasyon teknikleri ile elde edilmiş insan embriyolarının erken fertilizasyon ve klivaj dinamikleri yönünden morfokinetik analizi: Prospektif kardeş oosit çalışması

Abstract

Amaç: Bu çalışmada kendi gamet hücreleri kullanılarak Yardımcı Üreme Teknikleri (YÜT) tedavisi gören çiftlerde, inseminasyon için kullanılan In Vitro Fertilizasyon (IVF) veya Intra- Sitoplazmik Sperm Enjeksiyonu (ICSI) yönteminin embriyo morfokinetik gelişim zamanlarında bir değişiklik yapıp yapmadığının araştırılması amaçlanmıştır. Gereçler ve Yöntem: Çalışma Nisan 2014 – Ekim 2015 tarihleri arasında Bahçeci Fulya IVF Merkezinde gerçekleştirildi. Çalışmada, YÜT uygulanan 37 siklus dahil edildi. Her bir siklusta alınan oositler IVF ve ICSI adı ile rastgele ve mümkün olduğu ölçüde eşit sayıda iki gruba ayrıldı ve adı geçen yöntemle eş zamanlı olarak insemine edildi. Fertilizasyon kontrolü sonrasında oluşan tüm embriyolar time lapse inkübatör içinde inkübe edilerek, embriyo dondurma veya transfer aşamasına kadar olan süreçte morfokinetik kontrol noktalarına göre skorlandı. Bulgular: Dinamik embriyo gelişim parametreleri karşılaştırıldığında, takip başlangıç kriteri olarak inseminasyon zamanı alındığında, çalışmanın her iki kolunda olan embriyolar benzer klivaj dinamikleri ve kalite oranları gösterdiler. Pronukleus silinme zamanı takip başlangıç kriteri olarak alındığında ise, çalışma grupları arasında ekspanse blastokist aşamasına ulaşım süresi IVF ile insemine edilen embriyolar lehine anlamlı farklılık gösterdi (p<0.02). Sonuç: Kullanılan farklı inseminasyon teknikleri in vitro ortamda üretilen embriyoların morfokinetik davranışları üzerinde genel olarak anlamlı bir fark oluşturmamakla birlikte, uygun olgularda inseminasyon tekniği olarak IVF kullanılması, ekspanse blastokist aşamasına daha erken ulaşılmasını sağlaması açısından embriyo skorlama süreçlerinde göz önüne alınması gereken önemli bir bulgudur.

Morphokinetic analysis of early fertilization and embryo cleavage dynamics based on the mode of insemination technique used on human embryos: A prospective sibling oocyte study

Abstract

Purpose: The aim of this study was to assess whether in vitro fertilization(IVF) or intracytoplasmic sperm injection (ICSI) have different effects on the embryo’s morphokinetic development in couples undergoing assisted reproduction technologies (ART) treatment using their own gametes. Materials and Method: The study was conducted at the Fulya Bahceci IVF Centre between April2014 and October 2015. Thirty seven cycles ofART were included to the study. Oocytes obtained at each cycle were randomly selected toundergo fertilization through either IVF or ICSI. All embryos formed after fertilization were incubated inatime lapse incubator up until the time of embryo transfer or embryo freezing. The embryos were scored according to morphokinetic check points. Results: when the dynamic embryo development parameters were compared using the time of insemination as the starting point of follow up, embryos in both groups demonstrated similar cleavage dynamics. When the point at which the pronuclei disappeared was chosen as the starting point for follow up, the time to reach the expanded blastocyst stage was shorter in the group of oocytes fertilized through IVF(p<0.02). Conclusions: The use of different fertilization techniques does not appear to have a significant effect on the morphpkinetic behaviour of embryos developed in vitro. However, the use of IVF for insemination of oocytes in appropriate cases may allow the more rapid development of the embryo to the expanded blastocyst stage.
