Can systemic inflammatory indices be useful in hypertensive disorders of pregnancy?

Abstract

Aim: This study aimed to evaluate the effectiveness of systemic inflammatory indices in predicting and determining the prognosis of hypertensive disorders in pregnancy. Materials and Methods: This cross-sectional and retrospective single-center study involved 101 preeclampsia patients and 203 healthy pregnant women as the control group. Laboratory parameters were evaluated in the first trimester, at the time of diagnosis, and before delivery. The indices were calculated based on laboratory values: PLR (platelet/lymphocyte), APRI (AST/platelet), SII (neutrophil x platelet/lymphocyte) and SIRI (neutrophil x monocyte / lymphocyte). The study also analyzed subgroups such as early and late-onset preeclampsia, severe and non-severe preeclampsia, and HELLP syndrome. Results: The findings showed that indices calculated in the first trimester were insufficient in predicting preeclampsia, severe preeclampsia, early-onset preeclampsia, and HELLP syndrome. However, the PLR was lower in the preeclamptic group at the time of diagnosis due to changes in lymphocyte counts. The APRI was significantly higher in preeclamptic pregnancies, severe preeclampsia, early-onset preeclampsia, and HELLP syndrome in the pre-delivery period (p<0.001). Additionally, SIRI and PLR significantly changed before delivery in patients diagnosed with HELLP syndrome. Conclusion: These findings suggest that the inflammatory response may appear late but could be proportional to the prognosis of hypertensive disease in pregnancy; this holds true even in cases where systemic inflammatory indices are inadequate for predicting prognosis in the first trimester and at the time of diagnosis

Gebeliğin Hipertansif Hastalıklarında Sistemik İnflamatuar İndeksler Faydalı Olabilir Mi?

Abstract

Amaç: Sistemik inflamatuar indekslerin gebelikte hipertansif hastalıkların prognozunu tahmin etmede ve belirlemede faydalı olup olmadığını araştırmaktır. Gereç ve Yöntemler: Bu kesitsel ve retrospektif tek merkezli çalışma, çalışma grubu olarak 101 preeklampsi hastasını ve kontrol grubu olarak 203 sağlıklı hamile kadını içermektedir. Laboratuvar parametreleri, ilk trimesterde, tanı anında ve doğumdan önce olmak üzere üç ayrı dönemde değerlendirilmiştir. Araştırılan indeksler laboratuvar değerleri üzerinden hesaplanmıştır: PLR (trombosit/lenfosit), APRI (AST/trombosit), SII (nötrofil x trombosit / lenfosit), SIRI (nötrofil x monosit / lenfosit). Ayrıca söz konusu indeksler, çalışma grubu erken ve geç başlangıçlı preeklampsi, şiddetli ve şiddetli olmayan preeklampsi ve HELLP sendromunu içeren alt gruplar üzerinden analiz edilmiştir. Bulgular: İlk trimesterde hesaplanan tüm indeksler, preeklampsi, şiddetli preeklampsi, erken başlangıçlı preeklampsi ve HELLP sendromunu predikte etme konusunda yetersiz kalmıştır. Her ne kadar tanı anında hesaplanan indeksler de kötü prognoz öngörüsü konusunda yetersiz kalsa da, PLR preeklamptik grupta tanı anında kontrol grubuna göre daha düşük tespit edilmiştir. Bu farklılık lenfosit sayılarındaki değişiklikler nedeniyle ortaya çıkmıştır. Bununla birlikte doğumdan öncesi dönemde hesaplanan APRI, preeklamptik gebelerde, şiddetli preeklampside, erken başlangıçlı preeklampside ve HELLP sendromunda anlamlı derecede yüksek saptanmıştır (p<0.001). Ayrıca, HELLP sendromu teşhisi konulan hastalarda doğumdan önce SIRI ve PLR'nin de önemli ölçüde değiştiği bulunmuştur. Sonuç: Bu sonuçlar, inflamatuar yanıtın geç ortaya çıkabileceğini ancak gebelikte hipertansif hastalığın prognozu ile doğru orantılı olabileceğini önermektedir; bu durum, sistemik inflamatuar indekslerin prognozu ilk trimesterde ve tanı anında tahmin etmede yetersiz olduğu durumlarda bile geçerlidir.
