Predictive Value of First-Trimester Inflammatory Hematological Indices for Gestational Hypertension

Abstract

Objective: This study aimed to evaluate the predictive value of first-trimester hematological and inflammatory indices derived from complete blood count (CBC) parameters for gestational hypertension (GHT). Methods: A retrospective observational study was conducted on 80 pregnant women followed between January 2019 and December 2024. Participants were divided into two groups: women who remained normotensive (control, n=40) and those who developed isolated GHT after 20 weeks (GHT, n=40). First-trimester CBC results (≤14 weeks) were analyzed. Inflammatory indices, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), and systemic immune-inflammation index (SII), were calculated. Group comparisons were performed using t-tests or Mann–Whitney U tests. Receiver operating characteristic (ROC) analysis assessed diagnostic performance, and logistic regression identified independent predictors. Results: Women with GHT had significantly higher WBC, neutrophil, monocyte, hemoglobin, hematocrit, PDW, RDW, NLR, PLR, MLR, and SII values compared with controls (all p<0.05), while lymphocyte counts were lower (p=0.046). Platelet counts showed no difference (p=0.453). ROC analysis demonstrated that NLR (AUC=0.871, cut-off=2.14, sensitivity 78%, specificity 82%), RDW (AUC=0.787, cut-off=12.7, sensitivity 73%, specificity 77%), and SII (AUC=0.836, cut-off=513, sensitivity 83%, specificity 75%) had the strongest predictive value for GHT (all p<0.001). Logistic regression confirmed NLR, RDW, and PDW as independent predictors. No significant differences were observed for gestational age at delivery or birth weight, but neonates of hypertensive mothers required longer NICU stays (p=0.025). Conclusion: First-trimester inflammatory indices, particularly NLR, RDW, and PDW, represent independent predictors of gestational hypertension. CBC-derived parameters, being simple and widely accessible, may assist in early identification of women at risk for hypertensive disorders of pregnancy.
