İntrensek Sfinkter Yetmezliği Nedeniyle Tension Free Vaginal Tape Uygulanan Hastaların 5 Yıllık Sonuçları
Abstract
AMAÇ: ISD (İntrensek sfinkter yetmezliği) nedeniyle TVT (Tension free vaginal tape) yapılan hastaların uzun dönemdeki cerrahi başarı oranlarının değerlendirilmesi ve ayrıca cerrahinin başarısız olduğu grubun değerlendirilerek cerrahi başarısızlığa yol açan faktörlerin saptanması.
MATERYAL METOD: 2014 Ocak-2016 Aralık tarihleri arasında Zekai Tahir Burak Kadın Hastalıkları ve Doğum Hastanesi Jinekoloji Kliniği’nde ISD nedeniyle TVT uygulanmış 129 hasta dosyası incelenmiştir. 38 hasta çalışmaya dahil edilmiştir. Tüm hastalar aynı ekip tarafından preoperatif ve postoperatif olarak jinekolojik muayene, , Q tip test, öksürük stres test ve yaşam kalite ölçeklerinin kısaltılmış formları ile değerlendirlmiştir.. İstatistiksel analiz için SPSS 22 (Statistical Package for Social Sciences) programı kullanılmıştır.
BULGULAR: Çalışmaya dahil edilen hastaların postoperatif UDI-6 ve IIQ-7 sonuçları (sırasıyla 3.11±1.37 ve 3.08±1.25), preoperatif sonuçları (sırasıyla 13.72±3.15 ve 16.25±3.41) ile karşılaştırıldığında anlamlı şekilde daha düşüktü. (p<0.001) Ayrıca postoperatif dönemde öksürük stres test sıklığı (% 13.2), preoperatif (% 100) döneme göre anlamlı şekilde daha az sıklıktaydı. (p<0.001). Tariflediğimiz tanım doğrultusunda hastaların % 73.7’sinde kür, %13.2’sinde iyileşme saptandı. Multipl regresyon analizi, preoperative düşük MUSCP, düşük VLPP ve üretral hipermobilitenin olmayışını cerrahi başarısızlığı için bağımsız risk faktörleri olarak gösterdi. Eşik olarak belirlenen en iyi değerler ise MUCP için 15,5 cm H2O (%80 sensitivite ve % 67,8 spesivite) ve VLPP için 30.0 cmH2O’dur (% 80 sensitivite ve % 71,4 spesivite).
SONUÇ: TVT, ISD hastalarında etkin bir tedavi yöntemidir. Preoperatif MUCP değerinin <15.5 cmH2O ve/veya preoperative VLPP değerinin <30 cmH2O olması %80 oranla cerrahinin başarısız olacağını öngördürmektedir. Bu hastalar cerrahiden önce bu yönde bilgilendirilmelidir.

5 Year Outcomes of the Patients who went Through Tension Free Vaginal Tape Operation due to Intrinsic Sphincteric Deficiency
Abstract
OBJECTIVE: To evaluate 5-year outcomes of the patients who had TVT (Tension Free Vaginal Tape) due to ISD (Intrinsic Sphincteric Deficiency) and to identify the predictors for sling failure
MATERIALS AND METHODS: Patient outcomes after TVT due to ISD were evaluated through a retrospective review of medical records. 38 patients who underwent TVT surgery for ISD at our hospital between January 2014 and December 2016 were included as the study subjects. Patients were evaluated by gynaecologic examination, Q-tip test, cough stress test, and short forms of quality-of-life questionnaires. All surgeries and evaluations were performed by the same surgical team. SPSS 22 is used for statistical analysis.
RESULTS: Patients’ postoperative quality of life questionnaire scores were significantly lower. (13.72±3.15 and 16.25±3.41. vs 3.11±1.37 3.08±1.25) (p<0.001). Also cough stress test ratio was decreased from 100 % to 13.2 %. (p<0.001). 73.7 % of our patients were considered cured while %26.4
Multiple regression analysis showed that lower MUCP (maximal urethral closure pressure), lower VLPP (Valsalva leak point pressure), and absence of urethral hypermobility are independent risk factors for surgery failure. 15, 5cmH2O MUCP (80 % sensitivity, 67,8 % specificity) and 30cm H2O VLPP (80 % sensitivity, 71,4% specificity) levels can be used in predicting surgery results.
CONCLUSION: Tension Free Vaginal Tape is an effective treatment for Intrinsic Sphincteric Deficiency but there is an 80% possibility of treatment failure if the patient has preoperative MUCP below 15.5 cmH2O and/or VLPP below 30 cmH2O. These patients should be informed before surgery.
