Predictors of Pregnancy Latency and Neonatal Outcomes After Late Emergency Cerclage Beyond 24 Weeks of Gestation

Abstract

Aim: This study aimed to evaluate clinical, sonographic, and inflammatory markers associated with pregnancy latency, delivery before 34 weeks, and adverse neonatal outcomes in singleton pregnancies undergoing late emergency cerclage beyond 24 weeks of gestation. Materials and Methods: This retrospective cohort study included 34 singleton pregnancies that underwent emergency transvaginal cerclage after 24+0 weeks of gestation. Clinical, sonographic, laboratory, procedural, and perinatal outcome data were recorded. The primary outcome was delivery before 34 weeks. Secondary outcomes included cerclage-to-delivery interval, delivery at or beyond 32 and 34 weeks, adverse neonatal composite outcome, and individual neonatal outcomes. The predictive performance of cervical length and inflammatory indices was assessed using receiver operating characteristic curve analysis. Results: The median cerclage-to-delivery interval was 75.0 days. Delivery at or beyond 34 weeks occurred in 24 patients (70.6%), and adverse neonatal composite outcome was observed in 14 patients (41.2%). Patients delivering before 34 weeks had shorter cervical length, more frequent intra-amniotic sludge, and higher PLR, CLR, and CAR values. Cervical length showed the highest discriminatory performance for predicting delivery before 34 weeks (AUC=0.852; cut-off ≤14.0 mm). Conclusion: Late emergency cerclage beyond 24 weeks may provide meaningful pregnancy prolongation in selected singleton pregnancies. Residual cervical length, intra-amniotic sludge, and routine inflammatory indices may help identify patients at increased risk for early preterm birth and adverse neonatal outcomes.
