Evaluation Of Clinical Outcomes Of Hysteroscopic Isthmoplasty

Abstract

Aim: Isthmocele is a resevoir like pouch defect on the previous cesarean scar region. Retention of blood in this region is mostly related with postmenstrual bleeding, suprapubic pain, dysmenorrhea and secondary infertility. Hysteroscopic approach is minimal invasive and practical treatment which has been recently applied. However clinical results of hysteroscopic isthmoplasty is limited. The aim of this study is to evaluate the resolution of symptoms and clinical pregnancy rates after hysteroscopic isthmoplasty.Material and Methods: Hysteroscopic treatment results of patients with isthmocele who were admitted to Izmir Medicalpark Hospital between November 2015 and May 2018 were retrospectively evaluated. A total of twenty-five patients were included in the study. Hysteroscopic resection of isthmocele was performed with non-touch technique using bipolar Quick-Lock system 0 degree telescope Tonterra Medizintecknick GmbH, Stutgard, Germany The istmocele edges were resected with a cutting loop and pure cutting current placing its wall in continuity to that of cervical canal. The patients were evaluated with transvaginal ultrasonography one month after the procedure. A symptom score was established for each patient from 0 to 5 with symptoms including postmenstrual bleeding, bloody infected vaginal discharge, suprapubic coital pain and secondary infertility. Postoperative clinical pregnancy results and symptom scores were evaluated.Results: The mean age of patients was 35.5 min:28 max:45 . Symptom score was improved in 16 %64 patients after operation. Transvaginal ultrasonographic evaluation revealed that improvement was over 80% in 16 64% patients, 50-80% in 6 24% patients and 30-50% in 3 12% patients. Embryo transfer was performed in 12 patients. Ongoing pregnancy rate was 41.6% 5 patients . Spontaneous pregnancy was achieved in 2 patients. There was two cervical stenosis after the procedure. Cervical dilatation was sufficient to first patient. However hysteroscopic and laparascopic surgery was applied to the second patient.Conclusion: According to our study results the symptom scores and clinical preganacy rates were improved after hysteroscopic isthmoplasty. However larger studies are needed for evaluating the complications and long-term results of this treatment.

Histeroskopik İstmosel Onarımı Sonrası Klinik Sonuçların Değerlendirilmesi

Abstract

Giriş ve Amaç: İstmosel eski sezeryan skarında oluşan rezervuar benzeri doku defektidir. Kanın bu alanda birikimi sonucunda menstruasyon sonrası lekelenme, suprapubik ağrı, dismenore ve sekonder infertilite gelişe�- bilmektedir. İstmosel tedavisinde histeroskopik yaklaşım minimal invaziv ve pratik olması nedeniyle yakın zamanda uygulanmaya başlanmıştır. Fakat histeroskopik istmoplastinin klinik sonuçları ile ilgili veriler kısıtlıdır. Bu çalışmanın amacı kliniğimizde histereskopik istmosel onarımı uygulanan hastaların semptomlarındaki düzelme ve klinik gebelik oranlarını değerlendirmektir.Gereç ve Yöntemler: Kasım 2015 ve Mayıs 2018 tarihleri arasında İzmir Medikalpark Hastanesi Tüp Bebek Ünitesine başvuran ve histeroskopik istmosel onarımı uygulanan hastaların kayıtları retrospektif olarak incelenmiştir. Çalışmaya toplam 25 hasta dahil edilmiştir. İstmosel onarımı, serviks tenekulum ile tutulmadan ve dilate edilmeden bipolar Quick-Lock sistem 0 derece teleskop Tonterra Medizintecknick GmbH, Stutgard, Germany ile yapılmıştır. Istmosel kenarları bipolar ball elektrot ve bipolar rezektoskop kullanılarak servikal kanala ağızlaştırılmıştır. Hastalar operasyondan bir ay sonra transvaginal USG ile değerlendirilmiştir. Hastalara menstruasyon sonrası lekelenme, kanlı enfekte vajinal akıntı, suprapubik koital ağrı, sekonder infertilite sorgulaması ile 0-5 puanlık semptom skorlaması yapılmıştır. Hastaların tedavi sonrası semptom skorları ve gebelik oranları değerlendirilmiştir.Sonuçlar: Hastaların yaş ortalaması 35.50 min:28 maks:45 idi. Hastaların 16 %64 ’ ünde transvaginal ultrasonografik incelemede %80 ve üstünde düzelme, 6 %24 ’ünnde %50-80 arasında düzelme, 3 %12 ’sinde %30-50 arasında düzelme saptanmıştır. 16 %64 hastada semptom skorunda belirgin azalma saptanmıştır. Bu hastalardan 12 hastaya embryo transferi yapılmış olup 5 %41.6 ’inde devam eden gebelik elde edilmiştir. 2 olguda ise spontan gebelik elde edilmiştir. 2 olguda servikal stenoz gelişmiştir. İlk hastada servikal dilatasyon yeterli olmuştur. İkinci hastaya ise tekrar histeroskopik ve laparaskopik cerrahi uygulanmıştır.Sonuç: Sonuçlarımıza göre histeroskopik istmosel onarımı sonrasında hastaların semptom skorlarında belirgin azalma olduğu ve klinik gebelik oranlarında da artış olduğu saptanmıştır. Fakat bu tedavinin komplikasyonlar ve uzun dönem sonuçlarını değerlendirmek için daha geniş çaplı çalışmalara ihtiyaç mevcuttur.
