Doğum Şeklini Tahmin Etmede Baş-Simfizis Mesafesi, Baş İlerleme Mesafesi Ve Kaput Suksadeneum Boyutunun Yeri

Abstract

Amaç: Biz bu çalışmamızda intrapartum ultrasonografi ile ölçülen ve doğum eylemi takibinde kullanılabilecek parametrelerden baş-simfizis mesafesi, baş ilerleme mesafesi ve kaput suksadeneum boyutunun doğum şekli ile ilişkini anlamayı amaçladık.Gereç ve Yöntemler: Termde, baş prezentasyonda, aktif doğum eyleminde ve membran rüptürü olan 146 gebe araştırmaya dahil edildi. Aktif doğum eylemi, 10 dakika içinde en az üç ağrılı uterin kontraksiyon toplam 200-250 Montevideo ünite ve >5-6 cm servikal dilatasyon olması şeklinde kabul edildi. İntrapartum ultrasonografi ile baş-simfizis mesafesi, baş ilerleme mesafesi ve kaput suksadeneum boyutu ölçülerek kaydedildi. Hemen sonrasında klinik vajinal muayene uygulandı ve not edildi. Gebe ve yenidoğan özellikleri, doğum şekli, ultrasonografik ve klinik veriler istatistiksel olarak analiz edildi.Bulgular: Vajinal doğum yapan gebelerin ortalama baş-simfizis mesafesi değeri 18,6 ± 5,25 milimetre, ortalama baş ilerleme mesafesi değeri 39,1 ± 9,44 milimetre, ortalama kaput suksadeneum değeri 6,59 ± 6,46 milimetre bulundu. Sezaryen doğum yapan gebelerin ortalama baş-simfizis mesafesi değeri 23,15 ± 5,88 milimetre, ortalama baş ilerleme mesafesi değeri 26,53 ± 8,79 milimetre, ortalama kaput suksadeneum değeri 15,61 ± 4,8 milimetre bulundu. Vajinal doğum yapan gebelere kıyasla sezaryen doğum yapan gebelerdeki bu farklılıklar istatistiksel olarak anlamlıydı sırasıyla p:0,004, p

Importance Of Head-Symphysis Distance, Head Progression Distance, And Caput Succedaneum Size In Estimating The Delivery Mode

Abstract

Aim: In this study, we aimed to understand the relationship between the delivery mode and the sonographic parameters which can be used in the follow-up of labor such as head-symphysis distance, head progression distance, caput succedaneum.Material and Methods: 146 term pregnant women who were in active phase of labor and had ruptured membranes with cephalic presentation were included in the investigation. Active phase of labor accepted as being >5-6 centimeters of cervical dilatation and at least three uterine contraction total >200 Montevideo units within 10 minutes. Head-symphysis distance, head progression distance and caput succedaneum size were measured via intrapartum ultrasound and recorded. After immediately clinical vaginal examination was performed and noted. Patient characteristics, mode of delivery, ultrasonographic and clinic data were analyzed.Results: The mean values of head-symphysis distance, head progression distance and caput succedaneum of pregnant women who had vaginal delivery were 18,6 ± 5,25 milimeters, 39,1 ± 9,44 milimeters and 6,59 ± 6,46 milimeters, respectively. The mean values of head-symphysis distance, head progression distance and caput succedaneum of pregnant women who had cesarean delivery were 23,15 ± 5,88 milimeters, 26,53 ± 8,79 milimeters and 15,61 ± 4,8 milimeters, respectively.
