Spontaneous Ping-Pong Fracture With Subarachnoid Hemorrhage

Abstract

Depressed skull fractures in newborns are seen in 1/10000 live births. These rare fractures are often referred to as ‘ping-pong’ since they reflect the skull being transformed from convex to concave, due to its malleability from not being ossified. It accompanies generally to instrumental vaginal delivery, whereas spontaneous intrauterine ping-pong fractures in newborns delivered by cesarean section is a distinctly rare condition. Intracranial lesions are likely to occur from instrumental delivery, whereas there existed no report of spontaneous ping-pong fractures with intracranial lesions. Here, we reported a neonate who was born to cesarean section and presented a depressed ping-pong fracture with subarachnoid hemorrhage. Clinicians should carefully perform physical examination of newborns for skull abnormalizes. Newborns with spontaneous ping-pong fracture should be followed with close neurologic examination. Cranial imaging should be performed regardless of presence of neurologic signs.

Spontan Deplase Ping-Pong Kırığı Ve Subaraknoid Kanama

Abstract

Yenidoğanlarda kafatası kemiklerinde çökme kırıkları 1/10.000 oranında bildirilmiştir. Bu tür kırıklar, yenidoğanların ossifiye olmamış kemiklerinin konveks şeklinden konkav şekline dönüşmesi nedeniyle “ping-pong kırıkları” olarak adlandırılır. Sıklıkla müdahaleli doğum sırasında görülür. Anca sezaryen ile doğan olgularda spontan ping-pong kırıkları vaka takdimleri şeklinde rapor edilmiş olup eşlik eden intrakranyal patoloji bildirilmemiştir. Burada spontan ping-pong kırığına belirgin subaraknoid kanamanın eşlik ettiği sezaryen ile doğmuş bir yenidoğan sunulmaktadır. Müdahalesiz vajinal doğum veya sezaryen ile doğan yenidoğanların doğum odası baş boyun muayeneleri kafatası kemiklerinde çökme kırığı açısından dikkatli yapılmalıdır. Spontan ping-pong kırığı olan olgularda da intrakranyal patoloji olabileceği akılda tutulmalı ve kranyal görüntüleme yapılarak, yakın nörolojik muayene ile takip edilmelidir.
